吉西他滨联合顺铂新辅助化疗方案治疗局部晚期膀胱癌疗效观察

Therapeutic effect of gemcitabine combined with cisplatin neoadjuvant chemotherapy in the treatment of locally advanced bladder cancer

目的探讨局部晚期膀胱癌患者行吉西他滨联合顺铂新辅助化疗的临床效果。方法选取2016年3月～2018年3月我院收治的54例局部晚期膀胱癌患者作为研究对象,随机分为对照组27例及观察组27例。对照组患者行常规对症与顺铂治疗,观察组患者行吉西他滨联合顺铂新辅助化疗,应用统计学软件对两组患者临床疗效、不良反应、复发率、死亡率、生存期及治疗前后肿瘤直径变化情况进行比较。结果观察组患者临床疗效及生存期相比对照组更高,复发率及死亡率相比对照组更低,差异有统计学意义(P <0.05);两组患者治疗前肿瘤直径数据比较差异无统计学意义(P> 0.05),治疗后观察组患者肿瘤直径显著小于对照组,数据差异有统计学意义(P <0.05);两组患者不良反应发生率比较差异无统计学意义(P> 0.05)。结论局部晚期膀胱癌患者术前行吉西他滨联合顺铂新辅助化疗效果理想,可有效提升患者临床疗效,扩大手术适应证,对患者具有积极影响,因此可在临床中针对该种化疗方案进行推广。

Objective To explore the clinical effect of gemcitabine combined with cisplatin (neoadjuvant) chemotherapy in the treatment of locally advanced bladder cancer. Methods 54 patients with locally advanced bladder cancer admitted to our hospital from March 2016 to March 2018 were selected as the study subjects. They were randomly divided into control group (27 cases) and observation group (27 cases). Patients in the control group were treated with routine symptomatic treatment and cisplatin, while patients in the observation group were treated with gemcitabine combined with cisplatin neoadjuvant chemotherapy. The clinical efficacy, adverse reactions, recurrence rate, mortality rate, survival period and the changes of tumor diameter before and after treatment were compared between the two groups by statistical software. Results The clinical curative effect and survival period of the observation group were higher than the control group, the recurrence rate and mortality rate were lower than the control group, and the differences were statistically significant (P < 0.05);there was no significant difference between the two groups in the data of tumor diameter before treatment (P > 0.05);after treatment, the diameter of the tumors in the observation group was significantly smaller than the control group, and there was statistical significance (P < 0.05);there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Preoperative neoadjuvant chemotherapy with gemcitabine and cisplatin for patients with locally advanced bladder cancer is effective. It can effectively improve the clinical efficacy, expand the indications of surgery and have a positive impact on patients. Therefore, it can be popularized in clinic.
