三阴性乳腺癌患者3.0T MRI和钼靶的诊断结果与其病理特征分析

Diagnostic results and pathological features of 3.0T MRI and mammography in patients with triple negative breast cancer

目的分析三阴性乳腺癌患者3.0T MRI和钼靶的诊断结果及其病理特征。方法选取2016年10月～2018年10月医院收治的经手术病理证实的80例乳腺癌患者为研究对象,根据术后免疫组化染色,分为三阴性乳腺癌患者40例(TNBC组)和非三阴性乳腺癌患者40例(non-TNBC组)。分析病灶的钼靶和MR影像学表现及临床病理特征。结果 TNBC组在钼靶表现中,肿块形态多为圆形和椭圆形,且边缘清晰,少见毛刺;TNBC组在MRI表现中,肿块形态多为圆形和椭圆形,且边缘清晰,少见毛刺,肿块后方回声表现多为增强或无变化。TNBC组患者的淋巴结转移率明显高于非TNBC组,差异有统计学意义(P <0.05)。结论钼靶X线、3.0T MRI影像表现及临床病理特征有助于诊断三阴性乳腺癌。

Objective To analyze the diagnosis results and pathological features of 3.0T MRI and mammography in patients with triple negative breast cancer. Methods 80 cases with breast cancer confirmed by surgery and pathology in our hospital from October 2016 to October 2018 were selected as the study subjects.According to postoperative immunohistochemical staining,the patients were divided into triple negative breast cancer with 40 cases (TNBC group) and non-triple negative breast cancer with 40 cases (non-TNBC group).Mammography and MR imaging manifestations and clinicopathological features of the lesions were analyzed. Results In the mammography performance of the TNBC group,the shape of the mass were mostly round and elliptical,with clear edges and few burrs.In the MRI manifestations of the TNBC group,the shape of the mass were mostly round and elliptical,with clear edges and few burrs,the echoes behind the mass were mostly enhanced or unchanged.The lymph node metastasis rate in TNBC group was significantly higher than the non-TNBC group,the difference was statistically significant (P < 0.05). Conclusion Mammography X-ray,3.0 T MRI imaging and clinicopathologic features are helpful in the diagnosis of triple negative breast cancer.
