高频彩色多普勒超声引导颈内静脉穿刺置管的应用价值

Application value of high frequency color doppler ultrasound guided internal jugular vein intubation

目的探究经高频彩色多普勒超声(简称高频彩超)引导下实施颈内静脉穿刺置管术的效果。方法选取2017年3月～2018年3月在我院接受颈内静脉穿刺置管术治疗的患者共36例作为研究对象,以随机数字表法为依据进行分组,其中对照组18例,观察组18例。两组患者分别根据解剖标志下以及高频彩超引导下实施颈内静脉穿刺置管术,通过统计学软件计算并分析两组患者一次置管成功率、置管操作时间以及置管术后并发症情况,同时为患者行视觉模拟评分法(VAS)评定两组患者疼痛情况。结果观察组患者一次置管成功率相较于对照组显著更高,差异有统计学意义(P <0.05);观察组患者置管操作时间、VAS评分以及术后并发症发生率相较于对照组显著更低,差异有统计学意义(P <0.05)。结论高频彩超引导下实施颈内静脉穿刺置管术准确率及安全性相比传统解剖标志下置管术更高,有利于规避置管对患者机体组织造成的损伤,可有效降低患者痛苦感受,因此该种置管方法值得在临床中进行推广和应用。

Objective To explore the effect of performing internal jugular vein intubation guided by high frequency color Doppler ultrasound (referred to as high-frequency color Doppler ultrasound). Methods 36 patients who underwent internal jugular vein intubation in our hospital from March 2017 to March 2018 were selected as research objects.According to the random number table method,they were divided into the control group and the observation group, with 18 cases in each group.Patients of two groups underwent internal jugular vein intubation under anatomical landmarks and high-frequency color Doppler ultrasound.The success rate of one-time catheterization, the time of catheterization and the postoperative complications were calculated and analyzed by statistical software.The pain of patients in the two groups was assessed by visual analog scale (VAS). Results The success rate of one-time catheterization was significantly higher in the observation group than in the control group.The data between the groups were significantly different and the difference was statistically significant (P < 0.05).The catheterization time,VAS score,and postoperative complication rate in the observation group were significantly lower than those in the control group. Conclusion The accuracy and safety of the internal jugular vein intubation under the guidance of high frequency color Doppler ultrasound are higher than those under anatomical landmarks.It is conducive to avoiding the damage caused by the catheter on the patient's body tissue and can effectively reduce the patient's painful feeling.Therefore,this kind of catheterization method is worthy of promotion and application in the clinical practice.
