改良超声弹性成像评分标准在乳腺肿块鉴别诊断中的价值 被引量:2

摘要 目的研究改良超声弹性成像评分标准对乳腺肿块的临床鉴别价值。方法选择我院2014年1月~2017月12月收治的291例乳腺肿块患者进行研究,均进行超声弹性成像旧评分标准以及改良超声弹性成像评分标准进行评估,以手术病理为诊断金标准,分析改良超声弹性成像评分标准对乳腺肿块鉴别的诊断价值。结果病理证实291例乳腺肿块患者中恶性病变89例(30.58%),良性病变202例(69.42%)。改良评分对乳腺肿块良、恶性病变诊断率可高达93.52%、84.27%,明显高于旧评分的87.83%、68.54%,差异有统计学意义(P <0.05)。改良评分对乳腺肿块诊断灵敏度、阳性预测值及阴性预测值均高于旧评分,差异有统计学意义(P <0.05)。两评分对乳腺肿块诊断特异度上比较,差异无统计学意义(P> 0.05)。结论改良超声弹性成像评分对乳腺肿块鉴别诊断价值高,恶性病变和良性病变诊断率高,值得临床应用及推广。 Objective To study the clinical differential value of modified ultrasound elastography scoring criteria for breast masses. Methods 291 patients with breast masses who were admitted and treated in our hospital from January 2014 to December 2017 were included in the clinical study.All patients were evaluated by the old ultrasound elastography scoring criteria and the modified ultrasound elastography scoring criteria.Surgical pathology was used as the diagnostic gold standard,and the diagnostic value of modified ultrasound elastography scoring criteria for the differential diagnosis of breast mass was analyzed. Results Pathology confirmed that there were 89 cases of malignant lesions (30.58%) and 202 cases of benign lesions (69.42%) among 291 patients with breast masses.The diagnostic rate of benign and malignant breast lesions by modified scoring was as high as 93.52% and 84.27%,significantly higher than 87.83% and 68.54% of the old scoring,(P < 0.05).The diagnostic sensitivity, positive predictive value and negative predictive value of the modified scoring were all higher than those of the old scoring (P < 0.05).There was no difference in the specificity of the two scores for the diagnosis of breast masses,(P > 0.05). Conclusion The ultrasound elastography scoring has high value in differential diagnosis of breast masses and the diagnosis rate of malignant lesions and benign lesions is high,which is worthy of clinical application and promotion.
