牙龈卟啉单胞菌感染致患者慢性牙周炎发病的可能机制研究 被引量:12

摘要 目的:探讨牙龈卟啉单胞菌感染致患者慢性牙周炎发病的可能机制。方法:选取行慢性牙周炎拔牙治疗的患者128例作为观察组,以同期进行健康体检的牙周健康的128例健康人员作为对照组。采用实时荧光PCR对两组牙龈卟啉单胞菌进行检测,于无菌条件下刮取超过1/3的牙周膜组织,部分细胞采用牙龈卟啉单胞菌ACTT33277进行感染,部分不进行感染处理。采用Western Blot对NLRP3蛋白进行检测,采用酶联免疫吸附法对白细胞介素-1β(IL-1β)与IL-18的表达情况进行检测。结果:观察组牙龈卟啉单胞菌阳性率为82.0%,显著高于对照组牙龈卟啉单胞菌阳性率(18.0%),差异有统计学意义(P<0.01);在5 mm、6 mm、7 mm的探诊深度时,观察组牙龈卟啉单胞菌浓度均显著高于对照组,差异有统计学意义(P<0.01);观察组进行人工牙龈卟啉单胞菌感染的牙周膜细胞的NLRP3、IL-1β与IL-18表达水平显著高于未进行人工牙龈卟啉单胞菌感染的牙周膜细胞,差异有统计学意义(P<0.01)。结论:牙龈卟啉单胞菌感染可能导致慢性牙周炎,NLRP3、IL-1β与IL-18的高表达可能参与其发病机制。 Objective:To explore the possible mechanism of chronic periodontitis caused by porphyromonas gingivalis infection. Methods:128 patients with chronic periodontitis were treated as observation group, and the control group consisted of 128 healthy periodontal health people who underwent physical examination at the same time.Porphyromonas gingivalis in two groups were detected by real-time fluorescent PCR.More than one third of the periodontal ligament tissue was scraped under sterile condition.Some cells were infected by Porphyromonas gingivalis ACTT33277,while others were not infected.Western Blot was used to detect NLRP3 protein,and enzyme-linked immunosorbent assay was used to detect the expression of IL-1 β and IL-18. Results:The positive rate of porphyromonas gingivalis in the observation group was 82.0%,which was significantly higher than that in the control group (18.0%),the difference was statistically significant ( P <0.01).The porphyromonas gingivalis concentration in the observation group was significantly higher than that in the control group at 5 mm,6 mm and 7 mm depth,the difference was statistically significant ( P <0.01).The expression levels of NLRP3,IL-1β and IL-18 in the periodontal ligament cells infected by Porphyromonas gingivalis in the observation group were significantly higher than those in the periodontal ligament cells infected by Porphyromonas gingivalis without artificial infection,the difference was statistically significant ( P <0.01). Conclusion:Porphyromonas gingivalis infection may lead to chronic periodontitis,and the high expression of NLRP3,IL-1beta and IL-18 may be involved in its pathogenesis.
