经尿道前列腺等离子双极电切术治疗良性前列腺增生的临床分析

Clinical Analysis of Transurethral Prostate Plasma Bipolar Electrotomy for Benign Prostatic Hyperplasia

目的分析良性前列腺增生患者采用经尿道前列腺等离子双极电切术治疗的临床疗效。方法回顾性分析该院2016年3月—2017年12月期间收治的78例前列腺增生患者临床资料,根据患者入院就诊顺序将所有患者随机分为实验组(39例,采用经尿道前列腺等离子双极电切术治疗)和对照组(39例,采用经尿道前列腺电切术治疗),对比两组患者术中出血量、术后导尿管留置时间、住院时间、术后残余尿(RUV)、最大尿流量(Qmax)、国际前列腺症状评分(IPSS)等评分以及术后并发症发生率等情况。结果实验组患者术中出血量明显少于对照组(P<0.05),实验组患者相对对照组而言,术后导尿管留置时间及住院时间明显缩短(P<0.05)。实验组患者术后RUV、IPSS评分均低于对照组(P<0.05),实验组患者术后Qmax评分明显高于对照组(P<0.05)。实验组术后出现膀胱痉挛尿失禁、膀胱痉挛、继发性出血、尿道口灼伤等不良反应发生率5.1%明显低于对照组25.6%(χ~2=14.122,P<0.05)。结论良性前列腺增生患者采用经尿道前列腺等离子双极电切术可取得良好的疗效,不仅有利于缓解患者临床症状,也可减少术后并发症的发生,可在临床上广泛推广应用。

Objective To analyze the clinical efficacy of transurethral plasma bipolar electrotomy in patients with benign prostatic hyperplasia.Methods The clinical data of 78 patients with benign prostatic hyperplasia admitted to our hospital from March 2016 to December 2017 were retrospectively analyzed.All patients were randomly divided into experimental group(39 cases,transurethral prostate plasma bipolar electrosurgical treatment)and the control group(39 cases,treated with transurethral resection of the prostate)according to the patient admission order and control group,comparing the intraoperative blood loss,postoperative catheter indwelling time,hospital stay,postoperative residual urine(RUV),the maximum urine flow(Qmax),the International Prostate Symptom Score(IPSS),and the incidence of postoperative complications.Results The intraoperative blood loss of the experimental group was significantly lower than that of the control group(P<0.05).Compared with the control group,the postoperative catheter indwelling time and hospital stay were significantly shorter(P<0.05).The RUV and IPSS scores of the experimental group were lower than those of the control group(P<0.05).The Qmax score of the experimental group was significantly higher than that of the control group(P<0.05).The incidence of adverse reactions such as bladder spasm incontinence,bladder spasm,secondary hemorrhage,and urethral orifice burn in the experimental group(5.1%)was significantly lower than that of the control group by 25.6%(χ^2=14.122,P<0.05).Conclusion Patients with benign prostatic hyperplasia can achieve good curative effect by transurethral plasma bipolar electrotomy.It is not only beneficial to alleviate clinical symptoms,but also reduce postoperative complications.It can be widely applied in clinic.
