标准外伤大骨瓣减压术治疗多发性颅内血肿的效果

Effect of standard large trauma craniectomy in the treatment of multiple intracranial hematoma

目的探讨标准外伤大骨瓣减压术治疗多发性颅内血肿的效果。方法选取我院2017年2～12月收治的88例多发性颅内血肿患者进行研究,按照区组随机法分为对照组与观察组,每组各44例。对照组予以常规开颅手术治疗,观察组应用标准外伤大骨瓣减压术进行治疗,对比两组患者的格拉斯哥昏迷量表(GCS)评分、颅内压、治疗效果及并发症总发生率。结果观察组治疗24 h及治疗7、30 d的GCS评分均高于对照组,差异有统计学意义(P<0.05)。观察组术后1、7 d的颅内压均低于对照组,差异有统计学意义(P<0.05)。观察组的恢复优良率高于对照组,死亡率低于对照组,差异有统计学意义(P<0.05)。两组的并发症总发生率比较,差异无统计学意义(P>0.05)。结论标准外伤大骨瓣减压术治疗多发性颅内血肿可有效改善患者意识障碍、颅内压,治疗效果确切。

Objective To investigate the effect of standard large trauma craniectomy in the treatment of multiple intracranial hematoma.Methods A total of 88 patients with multiple intracranial hematoma admitted to our hospital from February to December in 2017 were enrolled.The patients were divided into the control group and the observation group according to the randomized method of the block,each group had 44 patients.The control group received routine craniotomy,and the observation group was treated with standard large trauma craniectomy.The Glasgow coma scale(GCS)score,intracranial pressure,treatment effect and the total incidence rate of complications were compared between the two groups.Results The GCS scores of 24 h,7 d and 30 d after treatment of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The intracranial pressure at 1 d and the intracranial pressure at 7 d after operation in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The recovery excellent rate of the observation group was higher than that of the control group,and the mortality rate in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).There was no difference in the total incidence rate of complications between the two groups(P>0.05).Conclusion Standard large trauma craniectomy in the treatment of multiple intracranial hematoma can effectively improve the patients'consciousness disorder and intracranial pressure,and the treatment effect is exact.
