阴囊镜在睾丸附睾疾病诊断与治疗中的应用分析

目的探究阴囊镜应用于睾丸附睾疾病诊断与治疗中价值。方法方便选取该院2016年7月—2017年7月间收治的100例睾丸附睾疾病患者开展研究,患者经B超诊断后,进一步施以阴囊镜检查或阴囊镜行等离子电切,观察患者应用阴囊镜、超声及病理诊断的结果,并观察手术结果。结果在睾丸扭转诊断中,超声准确率为80.00%,阴囊镜准确率为100.00%,(χ~2=4.44,P<0.05);在睾丸肿块诊断中,阴囊镜符合率为90.00%,B超为68.75%,阴囊镜更优,两者比较差异有统计学意义(χ~2=11.03,P<0.05);患者术后未见鞘膜积液、血肿等并发症,无切口感染,附睾肿块患者应用阴囊镜行等离子电切,复检时未见复发,术后睾丸形态、大小、血供正常。手术时间(36.59±5.87)min,住院时间(3.64±1.02)d,经手术治疗,患者VAS评分有显著改善,前后对比,差异有统计学意义(t=5.46,P<0.05)。结论阴囊镜诊断对睾丸扭转及附睾肿块的准确率较好,治疗较为安全,且疗效较好,研究中未见术后复发案例,可推广应用。

Objective To explore the value of scrotoscopy in the diagnosis and treatment of testicular and epididymal diseases.Methods A total of 100 patients with testicular epididymis diseases admitted to our hospital from July 2016 to July2017 were convenient studied.After diagnosis by B-mode ultrasound,the patients were further examined by scrotoscopy or plasma electrotomy under scrotoscopy.The results of scrotoscopy,ultrasonography and pathological diagnosis were observed,and the surgical results were observed.Results In the diagnosis of testicular torsion,the accuracy of ultrasonography was80.00%,that of scrotoscopy was 100.00%,(χ^2=4.44,P<0.05);in the diagnosis of testicular mass,the coincidence rate of scrotoscopy was 90.00%,that of B-mode ultrasonography was 68.75%,and that of scrotoscopy was better,with statistical significance(χ^2=11.03,P<0.05);there were no complications such as hydrocele,hematoma,no incision infection and epididymoma after operation.Scrotoscopy was used for plasma electrotomy.No recurrence was found at the time of reexamination.The morphology,size and blood supply of testis were normal after operation.The operation time was(36.59±5.87)min,and the hospital stay was(3.64±1.02)d.After operation,the VAS score of the patients was significantly improved,compared with,the difference was statistically significant(t=5.46,P<0.05).Conclusion Scrotoscopic diagnosis of testicular torsion and epididymal mass is more accurate,safe and effective.There is no recurrence case in the study,which can be popularized and applied.
