摘要
目的探讨彩色多普勒评价过敏性紫癜患儿肾血流变化及其对早期肾损害的临床价值。方法选取2016年2月至2018年6月我院过敏性紫癜患儿40例,正常儿童40例,通过彩色多普勒诊断仪测量肾血流情况双肾收缩期最大峰值血流速度(Vmax)、舒张期谷值血流速度(Vmin)、阻力系数(RI)并测定尿常规及尿微量蛋白水平。结果过敏性紫癜患儿与肾主动脉和叶间动脉Vmax、Vmin、RI均高于正常对照组,差异有统计学意义;40例过敏性紫癜患儿中多普勒肾血流异常31例(77.50%),高于尿微量白蛋白31例(47.50%),尿常规14例(35.00%),差异无统计学意义。结论彩色多普勒检测肾内血流动力学改变可反应过敏性紫癜患儿肾脏损害情况,且敏感性高于尿微量白蛋白和尿常规。
Objective:To evaluate the clinical value of color Doppler in the evaluation of renal blood flow in children with allergic purpura and its early renal damage.Methods:From February 2016 to June 2018, 40 children with allergic purpura and 40 normal children were enrolled in our hospital.The maximum peak blood flow velocity (Vmax) of renal systolic blood pressure was measured by color Doppler.), diastolic blood flow velocity (Vmin), drag coefficient (RI) and urine routine and urine microprotein levels.Results: The Vmax, Vmin and RI of children with allergic purpura and renal aorta and interlobular artery were higher than those of normal control group, the difference was statistically significant ( P <0.05).40 children with allergic purpura were found in Doppler kidney.There were 31 cases (77.50%) with abnormal blood flow, 31 cases (47.50%) with urinary albumin, and 14 cases (35.00%) with urinary routine.The difference was not statistically significant ( P >0.05).Conclusion:Color Doppler detection of intrarenal hemodynamic changes can reflect renal damage in children with allergic purpura, and the sensitivity is higher than urine microalbumin and urine routine.
