地塞米松与曲安奈德分别配合氧氟沙星治疗慢性中耳炎的疗效观察

Efficacy of dexamethasone and triamcinolone acetonide combined with ofloxacin in the treatment of chronic suppurative otitis media

目的探究地塞米松与曲安奈德分别配合氧氟沙星应用于慢性化脓性中耳炎的治疗效果。方法选取2016年1月～2017年1月本院收治的慢性化脓性中耳炎患者74例为研究对象。根据治疗方式差异,随机将患者分为两组,每组37例。对照组行曲安奈德配合氧氟沙星治疗;观察组行地塞米松配合氧氟沙星治疗。对比两组临床疗效、听力改善情况、临床症状改善时间以及不良反应发生情况。以评估慢性化脓性中耳炎治疗中氧氟沙星与地塞米松的联合应用效果。结果观察组患者治疗总有效率94.6%,显著高于对照组患者67.6%(χ~2=4.472,P<0.05);观察组患者听力改善率97.3%,显著高于对照组患者(70.3%)(χ~2=4.365,P<0.05);观察组鼓膜充血消失、穿孔愈合、耳痛消退以及中耳积脓消失时间分别(6.9±4.3)d、(7.9±5.1)d、(5.7±3.7)d、(7.6±4.7)d,均显著对于对照组患者的(10.4±5.2)d、(15.4±6.5)d、(9.8±4.9)d、(13.8±5.8)d (P<0.05);观察组不良反应发生率8.1%,对照组13.5%,两组对比无显著统计学差异(χ~2=1.632,P>0.05)。结论地塞米松配合氧氟沙星治疗慢性化脓性中耳炎疗效优于曲安奈德配合氧氟沙星,更利于改善患者听力,缩短恢复时间,不良反应少,安全性高,值得推广应用。

Objective To explore the efficacy of dexamethasone and triamcinolone acetonide combined with ofloxacin in the treatment of chronic suppurative otitis media.Methods From January 2016 to January 2017,74 patients with chronic suppurative otitis media were selected.Patients were randomly divided into two groups according to the difference in treatment methods,with 37 cases in each group.The control group was treated with triamcinolone acetonide combined with ofloxacin and the study group was treated with dexamethasone combined with ofloxacin.The clinical efficacy,the improvement of hearing,the improvement time of clinical symptoms and the incidence of adverse reactions were compared between two groups to evaluate the combined effect of ofloxacin and dexamethasone in the treatment of chronic suppurative otitis media.Results The total effective rate of the treatment group was 94.6%,which was significantly higher than that of the control group (67.6%,χ^2=4.472,P<0.05);The improvement rate of hearing in the study group was 97.3%,which was significantly higher than that in the control group (70.3%)(χ^2=4.365,P<0.05);The study group of tympanic membrane perforation healing,congestion disappeared,earache subsided and middle ear empyema disappeared time (6.9± 4.3) d respectively,(7.9±5.1) d,(5.7±3.7) d,(7.6±4.7) d,significantly for the patients in the control group (10.4±5.2) d,(15.4± 6.5) d,(9.8±4.9) d,(13.8±5.8) d (P<0.05);The incidence of adverse reactions was 8.1% in the study group and 13.5% in the control group.There was no significant difference between the two groups (χ^2=1.632,P>0.05).Conclusion Efficacy of dexamethasone combined with ofloxacin is superior to triamcinolone acetonide combined with ofloxacin in the treatment of chronic suppurative otitis media.It is beneficial to improve the hearing of patients and shorten the recovery time and has less adverse reactions and high safety,which is worthy of popularization and application.
