经鼻导管湿化高流量吸氧治疗呼吸衰竭患者的临床研究 被引量:2

摘要 目的探讨经鼻导管湿化高流量吸氧治疗呼吸衰竭患者中的临床疗效。方法选取2014年7月至2017年10月我院收治的144例呼吸衰竭患者,按照随机数字表法将其分为两组(对照组72例和研究组72例),对照组患者予以无创正压通气治疗,研究组患者采取经鼻导管湿化高流量吸氧治疗,比较两组患者治疗后脱机天数、1个月插管率以及3个月时患者病死率。结果研究组患者脱机天数为(20.3±9.3)d,对照组患者脱机天数为(26.5±11.0)d,比较差异显著(P <0.05);研究组患者1个月插管率与对照组比较相近差异不显著(P> 0.05),而3个月时病死率方面研究组患者明显低于对照组(P <0.05)。结论与无创正压通气治疗相比,经鼻导管高流量吸氧在治疗呼吸衰竭上具有有效、安全及易耐受等特点,其值得推广借鉴。 Objective To investigate the clinical efficacy of nasal catheter humidification and high flow oxygen therapy in patients with respiratory failure. Methods 144 patients with respiratory failure admitted to our hospital from July 2014 to October 2017 were divided into two groups according to the random number table (72 in the control group and 72 in the study group), and the patients in the control group were given noninvasive positive pressure Ventilation treatment, the study group to take high-flow nasal catheter humidification oxygen treatment, two groups of patients after treatment compared to the number of days off, intubation rate of 1 month and 3 months of patient mortality. Results The number of off-line days was (20.3±9.3) days in the study group and (26.5±11.0) days in the control group (P <0.05). The intubation rate in the study group was significantly higher than that in the control group There was no significant difference between the two groups (P >0.05). The mortality of the study group at 3 months was significantly lower than that of the control group (P <0.05). Conclusion Compared with non-invasive ventilation, nasal cannula high-flow oxygen inhalation is effective and safe in the treatment of respiratory failure, which is worth to promote.
