经鼻导管湿化高流量吸氧治疗呼吸衰竭患者的临床研究

Therapeutic Effect of Transnasal Catheter Wetted High Flow Oxygen Inhalation in Patients with Respiratory Failure

目的探讨经鼻导管湿化高流量吸氧治疗呼吸衰竭患者中的临床疗效。方法选取2014年7月至2017年10月我院收治的144例呼吸衰竭患者,按照随机数字表法将其分为两组(对照组72例和研究组72例),对照组患者予以无创正压通气治疗,研究组患者采取经鼻导管湿化高流量吸氧治疗,比较两组患者治疗后脱机天数、1个月插管率以及3个月时患者病死率。结果研究组患者脱机天数为(20.3±9.3)d,对照组患者脱机天数为(26.5±11.0)d,比较差异显著(P <0.05);研究组患者1个月插管率与对照组比较相近差异不显著(P> 0.05),而3个月时病死率方面研究组患者明显低于对照组(P <0.05)。结论与无创正压通气治疗相比,经鼻导管高流量吸氧在治疗呼吸衰竭上具有有效、安全及易耐受等特点,其值得推广借鉴。

Objective To investigate the clinical efficacy of nasal catheter humidification and high flow oxygen therapy in patients with respiratory failure. Results The number of off-line days was (20.3±9.3) days in the study group and (26.5±11.0) days in the control group (P <0.05). The intubation rate in the study group was significantly higher than that in the control group There was no significant difference between the two groups (P >0.05). The mortality of the study group at 3 months was significantly lower than that of the control group (P <0.05). Conclusion Compared with non-invasive ventilation, nasal cannula high-flow oxygen inhalation is effective and safe in the treatment of respiratory failure, which is worth to promote.
