出院准备服务对脑卒中吞咽障碍患者康复效果和睡眠质量影响的研究

Study on Rehabilitation Effect of Stroke Patients with Dysphagia Based on the Concept of Discharge Preparatory Service

目的:探讨基于出院准备服务概念的护理干预对脑卒中后吞咽障碍患者吞咽康复功能恢复和睡眠质量的影响。方法:选取2017年5月至2018年5月福建中医药大学附属康复医院脑病康复科一科和二科收治的脑卒中后吞咽困难患者60例作为研究对象,随机分为观察组和对照组,每组30例。对照组进行常规护理,观察组在常规护理的基础上根据自制的出院准备服务计划表进行护理干预; 2组在入院时和干预1个月后分别进行吞咽疗效评定、住院天数、并发症、满意度调查及睡眠质量比较。结果:观察组总有效率高于对照组(P <0. 05),不良事件发生率低于对照组(P <0. 05),PSQI评分低于对照组(P <0. 05)。结论:脑卒中伴吞咽障碍患者行基于出院准备服务概念的护理干预护理后,可改善其吞咽功能及睡眠质量,降低不良事件发生率,效果显著,值得临床推广应用。

Objective:To explore the effect of nursing intervention based on the concept of discharge preparation service on the recovery of swallowing rehabilitation function and sleep quality in patients with dysphagia after stroke.Methods:A total of 60 patients were divided into control group and experimental group.The control group received routine nursing,and the experimental group received nursing intervention according to the self-made discharge preparation service schedule on the basis of routine nursing.At the time of admission and 1 month after intervention,the two groups were assessed for swallowing efficacy,length of stay,complications,satisfaction survey and sleep quality.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05),the incidence of adverse events was lower than that of the control group(P<0.05),and the PSQI score was lower than that of the control group(P<0.05).Conclusion:After nursing intervention based on the concept of discharge preparation service,patients with stroke and dysphagia can improve their swallowing function and sleep quality,and reduce the incidence of adverse events.
