血清内PGⅠ、PGⅡ、PGR水平对胃癌及Hp感染的临床诊断价值考察

Clinical diagnostic value of serum PGⅠ, PGⅡ and PGR levels in gastric cancer and Hp infection

目的探讨血清内胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、胃蛋白酶原Ⅰ和Ⅱ比值(PGR)水平对胃癌及幽门螺杆菌(Hp)感染的临床诊断价值。方法选取2017年1月～2018年6月收治的100例胃癌患者作为病例组,同期选取来院体检的100名健康体检者作为对照组。采用酶联免疫吸附实验(ELISA)检测受试者血清中PGⅠ、PGⅡ水平,计算PGI/PGⅡ(PGR)值,应用14C尿素呼气试验判断受试者Hp感染情况。结果胃癌患者血清内PGⅠ水平及PGR值显著低于健康体检者,差异具有统计学意义(P <0.05);两组受试者血清内PGⅡ水平比较差异无统计学意义(P> 0.05);胃癌患者Hp感染阳性率明显高于健康体检者,差异具有统计学意义(P <0.05);Hp感染阳性者PGR值低于Hp感染阴性者,PGR值由大至小排序为:对照组Hp感染阴性者>病例组Hp感染阴性者>对照组Hp感染阳性者>病例组Hp感染阳性者。结论血清内PGⅠ和PGR水平对胃癌伴Hp感染阳性者具有较高的诊断价值,对于Hp感染阳性者胃癌筛查具有重要意义。

Objective To investigate the clinical diagnostic value of serum levels of pepsinogen Ⅰ(PGⅠ),pepsinogen Ⅱ(PGⅡ),ratio of pepsinogen Ⅰ to Ⅱ(PGR) in gastric cancer and Helicobacter pylori(Hp) infection.Methods 100 patients with gastric cancer admitted to our hospital from January 2017 to June 2018 were selected as the case group,and 100 healthy people who came to our hospital for physical examination were selected as the control group during the same period.Enzyme-linked immunosorbent assay(ELISA) was used to detect the serum levels of PGⅠ and PGⅡ,then calculated the PGⅠ/PGⅡ(PGR) values.And 14C urea breath test was used to determine the Hp infection.Results The serum levels of PGⅠ and PGR in patients with gastric cancer were significantly lower than those in healthy people,with statistical significance(P<0.05);there was no significant difference in serum levels of PGⅡ between the two groups;the positive rate of Hp infection in patients with gastric cancer was significantly higher than that in healthy people,with statistical significance (P<0.05);The PGR value of the positive group was lower than that of the negative group.The order of the PGR value was: the negative group of Hp infection in the control group > the negative group of Hp infection in the case group > the positive group of Hp infection in the control group > the positive group of Hp infection in the case group.Conclusion Serum levels of PGⅠ and PGR have high diagnostic value for gastric cancer patients with HP infection,and are of great significance for screening gastric cancer patients with Hp infection.
