实施护理责任制对梗阻性黄疸患者经皮肝穿刺胆道引流术后胆道引流护理质量的作用分析

Effect of nursing responsibility system on nursing quality of biliary drainage after PTCD for patients with obstructive jaundice

目的探讨实施护理责任制对梗阻性黄疸患者经皮肝穿刺胆道引流术(PTCD)后护理质量的作用。方法选取从2015年4月～2018年4月某院收治的梗阻性黄疸需手术患者82例作为观察对象,按照随机分组法分成观察组与对照组。对照组予以常规护理,观察组则在常规护理的基础上实施责任制护理干预。分别比较两组在术后并发症发生情况、干预前后心理状态、以及干预前后生活质量差异。结果观察组术后胆道感染、腹腔内出血、休克以及恶心呕吐发生率明显低于对照组。干预后观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分明显低于对照组。干预后观察组心理状态评分明显优于对照组,差异均具有统计学意义(P <0.05)。结论实施护理责任制可显著提高梗阻性黄疸患者术后护理质量,缓解焦虑、抑郁等负性情绪,提高生活质量。

Objective To explore the effect of implementing nursing responsibility system on post-operative nursing quality of patients with obstructive jaundice.Methods 82 patients with obstructive jaundice treated in our hospital from April 2015 to April 2018 were selected as observation objects.According to the random grouping method,82 patients with obstructive jaundice were divided into two groups,the observation group and the control group.The control group was given routine nursing,and the observation group was given responsibility nursing intervention on the basis of routine nursing.The differences in postoperative complications,psychological state before and after intervention,satisfaction degree and quality of life before and after intervention between the two groups were compared.Results The incidence of biliary tract infection,intraperitoneal hemorrhage,shock and nausea and vomiting in the observation group was lower than that in the control group respectively(P<0.05);After intervention,the scores of SAS and SDS in the observation group were lower than those in the control group respectively(P<0.05);After intervention,the psychological state score of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The implementation of nursing responsibility system can significantly improve postoperative nursing quality of patients with cholangiocarcinoma,alleviate anxiety,depression and other negative emotions,improve satisfaction and quality of life.
