39例皮肤纤维瘤的临床和皮肤镜及病理学特征分析

Clinical, dermoscopic and pathological features of 39 cases of dermatofibroma

目的探讨皮肤纤维瘤的临床、皮肤镜和病理学特点。方法对39例皮肤纤维瘤患者的临床表现、皮肤镜和组织病理学检查进行回顾性分析。结果 39例患者中男性10例,女性29例;平均年龄(31±11.9)岁。临床表现特点为暗红色或褐色结节或丘疹,一般无压痛,四肢多见。皮肤镜检查:肿物中央有类似瘢痕的白色斑疹或似瘢痕样结构,周围可见棕色的色素网,肿物周围边界清楚。组织病理学特征:表皮指状延长,肿瘤细胞主要位于真皮中部,多数纤维瘤的肿瘤区与表皮之间有一薄层结缔组织间隔并基底层色素增多,肿瘤细胞呈细长梭形,交织状排列。结论皮肤纤维瘤的临床表现各阶段有所不同,病理检查可确诊,皮肤镜可辅助临床诊断,并对手术切除有一定指导作用。

Objective To investigate the clinical,dermoscopic,and pathological features of dermatofibroma.Methods The clinical manifestations,dermatoscope,and histopathology of 39 patients with dermatofibroma were analyzed retrospectively.Results The 39 patients,10 were male and 29 were female.The average age was(31±11.9)years.The typical clinical manifestations of this disease were dark red or brown nodules or papules,generally without tenderness,and were common on the limbs.Dermoscopy:The center of the tumor had a white macula or scar-like structure,with a brown pigment network around it and a clear boundary around the tumor.Histopathological features:the epidermis was fingered and the tumor cells were mainly located in the middle of the dermis.Most fibromas had a thin layer of connective tissue space between the tumor area and the epidermis and the pigment in the basal layer was increased.The tumor cells were elongated and shuttle-shaped and interwoven.Conclusions The clinical manifestations of dermatofibroma are different in different stages.The pathological examinations can be confirmed.Dermatoscopy can assist in clinical diagnosis and play a guiding role in surgical resection.
