化痰通络汤在脑梗塞恢复期偏瘫症中的临床应用研究

Clinical Application of Huatan Tongluo Decoction in Cerebral Infarction Convalescence Hemiplegia

目的:研究化痰通络汤联合在脑梗塞恢复期偏瘫症中的临床应用及其对患者血液流变学的影响。方法:选择2015年5月—2017年2月在本院进行康复治疗的100例脑梗塞恢复期偏瘫患者,采用随机数字表法随机均分为两组,每组各50例。对照组在恢复期采用舒血宁注射液静脉注射治疗;观察组在此基础上加服化痰通络汤进行治疗。比较两组治疗前后血液流变学指标情况、血脂指标变化情况,并对两组患者临床治疗效果进行分析比较。结果:两组患者在接受治疗后,血液黏度指标经检测均明显下降,而观察组全血黏度及血浆黏度与对照组比较有显著性差异(P <0. 05)。两组患者血浆纤维蛋白原、血小板及红细胞聚集指数均呈下降状态。观察组血浆纤维蛋白原、血小板及红细胞聚集指数明显低于对照组(P <0. 05)。两组患者经治疗后甘油三酯、总胆固醇、低密度脂蛋白水平明显降低,而高密度脂蛋白水平则明显增加。治疗后观察组与对照组比较,患者血脂TC、TG、LDL-C、HDL-C水平有统计学差异(P <0. 05)。观察组患者临床总有效率96. 00%明显高于对照组82. 00%,差异有统计学意义(P <0. 05)。结论:化痰通络汤辅助治疗脑梗塞恢复期偏瘫患者安全有效,能显著改善患者血液流变学指标。

Objective: To study the clinical application of Huatan Tongluo Decoction combined with cerebral infarction in the recovery period of hemiplegia and its influence on hemorheology. Methods: From May 2015 to February 2017,100 patients with hemiplegia at the convalescent stage of cerebral infarction in our hospital were randomly divided into two groups,50 cases in each group. The control group was treated with Shuxuening Injection in the recovery stage and the observation group was treated with Huatan Tongluo Decoction on this basis. Blood rheology indexes and blood lipid indexes were compared between the two groups before and after treatment,and the two groups’ clinical treatment effect was analyzed and compared. Results: The blood viscosity indexes of the two groups were obviously decreased after treatment,but the blood viscosity and plasma viscosity of the observation group were significantly different from those of the control group( P < 0. 05). The plasma fibrinogen,platelet and erythrocyte aggregation index decreased in the two groups. The plasma fibrinogen,platelet and RBC aggregation index of the observation group were lower than that of the control group( P < 0. 05). Triglyceride,total cholesterol and low-density lipoprotein levels were significantly lower in the two groups after treatment while HDL levels were significantly increased. After treatment,the observation group’s TC,TG,LDL-C and HDL-C levels were statistically significant compared with those of the control group( P < 0. 05).The total effective rate of the observation group was 96%,which was significantly higher than that of the control group( 82%) and the difference was statistically significant( P < 0. 05). Conclusion: It is safe and effective to treat hemiplegia patients with cerebral infarction at recovery stage with the help of Huatan Tongluo Decoction. It can significantly improve the hemorheological indexes of the patients.
