献血前丙氨酸氨基转移酶初筛阈值的确定

Determination of the preliminary screening threshold of alanine aminotransferase in blood donation

目的研究和确定流动献血丙氨酸氨基转移酶(Alanine aminotransferase,ALT)初筛阈值,减少血液报废。方法先应用干化学方法对血液样本进行初筛,对ALT水平>40 U/L且<50 U/L的样本使用全自动生化仪进行复检。对初筛值和复检值建立线性回归方程,计算出ALT初筛的阈值,并依此为基准计算出2017年新旧阈值对应的复检不合格率。结果通过对100份血液样本的分析,计算出线性回归方程y=1.046 4x-0.433,相关系数R值为0.905 3,ALT检测新阈值为47 U/L。以50 U/L为检测阈值,2017年上半年复检不合格率为0.020%;以47 U/L为检测阈值,2017年下半年复检不合格率为0.012%,有效降低了复检不合格率。结论针对不同仪器和试剂确定初筛ALT阈值,可以降低复检不合格率,减少血液报废。

Objective To study and determine the preliminary screening threshold of Alanine amtransferinoase (ALT) and reduce blood scrap rate.Methods The blood samples were firstly screened by dry chemical method,and the ALT level of the samples greater than 40 U/L and less than 50 U/L were re-examined by automatic biochemical analyzer.The linear regression equation was established with the initial screening value and the retest value,and the threshold value of ALT preliminary screening was calculated.Based on the new threshold value,the failure rate of reexamination in 2017 was calculated.Results Through the analysis of 100 blood samples,the linear regression equation y=1.046 4x-0.433 was calculated,the correlation coefficient R value was 0.905 3,and the new threshold value for ALT detection was 47 U/L.Taking 50 U/L as the test threshold,the failure rate of re-inspection in the first half of 2017 is 0.020%.Taking 47 U/L as the test threshold,the failure rate of re-inspection in the second half of 2017 is 0.012%,effectively reducing the failure rate of reinspection.Conclusion The determination of ALT threshold value of preliminary screening using different instruments and reagents can reduce the retest failure rate and blood scrap.
