经穴推拿治疗剖宫产术后缺乳的临床研究

Clinical Study of Meridian and Acupoint Massage in Treating Postpartum Hypogalactia After Cesarean Section

【目的】探讨经穴推拿对剖宫产术后缺乳产妇的影响。【方法】将60例行剖宫产术的产妇随机分为试验组和对照组,每组各30例。对照组产妇给予产科术后常规护理,试验组产妇在对照组的基础上根据不同的经型选取相关穴位及手法进行推拿按摩,比较2组产妇的泌乳量、乳房充盈度、乳汁粘稠度、乳汁淤积程度评分的变化情况,并评价2组的疗效。【结果】(1)研究过程中,试验组2例产妇因急性乳腺炎剔除,其余28例完成观察;对照组30例产妇全部完成观察,无脱落剔除病例。(2)研究的第1天,2组产妇的各项评分比较,差异均无统计学意义(P>0.05);从研究的第3天开始,试验组产妇的泌乳量、乳房充盈度、乳汁粘稠度、乳汁淤积程度评分均低于对照组,差异均有统计学意义(P<0.01)。(3)干预后,试验组的总有效率为96.43%,对照组为50.00%,组间比较(卡方检验),试验组的疗效优于对照组,差异有统计学意义(P<0.01)。【结论】经穴推拿可有效促进剖宫产术后缺乳产妇的泌乳量,减轻涨奶程度,具有显效快、疗效确切的优势。

Objective To investigate the influence of meridian and acupoint massage on postpartum hypogalactia after cesarean section. Methods A total of 60 puerperant with postpartum hypogalactia were randomly divided into trial group and control group,30 cases in each group. Both groups were given conventional postpartum nursing,and additionally the trial group was given meridian and acupoint massage based on the syndrome differentiation.The scores of lactation volume,breast filling,milk viscosity,and galactostasis of the two groups were compared,and the clinical efficacy of the two groups was evaluated after treatment. Results(1)During the trial,2 cases from the trial group were excluded for suffering from acute mastitis,and a total of 28 cases in the trial group completed the trial. All cases of the control group completed the trial.(2)On the first trial day,the differences of the scores of all observation indexes were insignificant between the two groups(P>0.05);from the third trial day,the scores of lactation volume,breast filling,milk viscosity,and galactostasis in the trial groups were lower than those in the control group,the differences being significant(P<0.01).(3)After intervention,the total effective rate in the trial group was 96.43% and that in the control group was 50.00%,and the results of Chi-square test showed that the therapeutic efficacy of the trial group was stronger than that of the control group, and the difference being significant(P<0.01). Conclusion Meridian and acupoint massage is effective on increasing lactation volume and decreasing breast filling, with the advantages of being fast in starting an effect and having certain therapeutic efficacy.
