摘要
目的通过该研究找到一种可替代自体皮移植术治疗糖尿病足溃疡的新方法,提高糖尿病足溃疡愈合质量,减少愈合后瘢痕增生,防止二次复发。方法选择2016年6月至2018年5月收治的40例糖尿病足患者,按照随机数字表法分为自体皮移植组20例和组织工程皮肤移植组20例。两组患者分别于移植术后观察创面愈合时间,术后3个月、6个月后对于植皮区皮肤瘢痕进行评价。对数据行t检验、X^2检验。结果自体皮移植组术后于(15.12±3.24)天创面愈合,组织工程皮肤组术后(16.23±3.15)天创面愈合。两者差异无统计意义学上意义(P>0. 05)。自体皮移植组植皮区创面愈合后3个月及6个月皮肤瘢痕评分(温哥华瘢痕量表评估标准(Vancouver scar scale即VSS标准)分别为(12.32±1.36)分和(13.29±1.68)分。组织工程皮肤移植组分别为(9.32±1.54)分和(10.83±1.59)分。两者差异有统计意义学上意义(P<0. 05)。结论组织工程皮肤是治疗糖尿病足创面的有效方法,在部分情况下可替代自体皮移植术治疗糖尿病足。
Objective To find a new method of treating diabetic foot ulcer by using autogenous skin grafting,to improve the healing quality of diabetic foot ulcer,to reduce scar hyperplasia after healing and to prevent secondary recurrence.Methods From June 2016 to May 2018,40 patients with diabetic foot were randomly divided into two groups:autogenous skin transplantation group ( n=20) and tissue engineered skin transplantation group ( n=20).The wound healing time after transplantation and the evaluation of skin scar in skin graft area after 3 months and 6 months were observed in both groups.Data row t test,x 2 test.Results The wound healing was (15.12±3.24) days in the autogenous skin graft group and (16.23±3.15) days in the tissue engineered skin group.There is no statistical difference between the two.In the autogenous skin graft group,the skin scar score (Vancouver scar scale) was 12.32±1.36 and 13.29±1.68 respectively at 3 and 6 months after healing.The score of tissue engineered skin transplantation was (9.32±1.54) and (10.83±1.59),respectively.There is a statistical difference between the two.Conclusion tissue engineered skin is an effective method for treating diabetic foot wounds.
