左心房容积追踪技术评价系统性红斑狼疮患者左心房功能

摘要 目的:应用左心房容积追踪技术(Left atrial volume tracing technique,LAVT)评价系统性红斑狼疮(systemic lupus erythematosus,SLE)患者左心房功能,探讨其临床应用价值。方法:选择SLE患者32例设为SLE组,正常对照组30例,应用LAVT技术获取左心房最大容积(LAVmax)、心电图p波对应容积(LAVp)、左心房最小容积(LAVmin)、左心房储存容积(LAVtotal);以上均用体表面积(BSA)标准化得到LAVImax、LAVIp、LAVImin、LAVItotal,并计算左房主动搏出量(atrial active stroke volume,AA-SV)、左心房扩张指数(Left atrial dilatation index,LAEI)等参数。结果:与对照组比较,SLE组LAVImax、LAVIp、LAVImin均升高(P<0.01);LAEI、AA-SV升高(P<0.05)。结论:LAVT技术操作简便、重复性高,能够早期发现SLE患者左心房功能的变化,表现为左心房储存功能、辅助泵血功能增强。 Objective:To explore the clinical application value of left atrial volume tracing technique(LAVT)in evaluating left atrial function of patients with systemic lupus erythematosus(SLE).Methods:32 patients with SLE were selected as SLE group and 30 patients in the normal control group.LAVT technology was used to obtain the maximum volume of left atrium(LAVmax),the corresponding volume of electrocardiogram p wave(LAVp),the minimum volume of left atrium(LAVmin),and the storage volume of left atrium(LAVtotal).Above all was standardized to obtain LAVImax,LAVIp,LAVImin and LAVItotal by body surface area(BSA),and parameters such as atrial active stroke volume(aa-sv)and Left atrial dilatation index(LAEI)were calculated.Results:Compared with the control group,LAVImax,LAVIp and LAVImin in SLE group were all increased(P<0.01).LAEI and aa-sv also increased(P<0.05).Conclusion:LAVT technique is simple to operate and highly repeatable,and can detect early changes in left atrial function in SLE patients,manifested as enhanced left atrial storage function and auxiliary pumping function.
