外周血T、B、NK细胞亚群与HBV患者核酸复制关系的研究

摘要 目的:研究外周血T、B、NK细胞亚群与乙型肝炎病毒(HBV)感染患者核酸复制的关系。方法:2017年7月至2018年4月收治的HBV感染患者74例为观察组,另取同期进行体检的健康人员50例为对照组,分别比较两组人员外周血T、B、NK细胞亚群水平,将观察组患者按照HBV-DNA载量水平的不同分为HBV-DNA阴性组20例,HBV-DNA低拷贝组30例,HBV-DNA高拷贝组24例,比较3组患者外周血T、B、NK细胞亚群水平并作相关性分析。结果:观察组外周血CD3^+、CD3^+CD4^+、CD3-CD56^+水平比对照组低(P<0.05),而CD19^+水平比对照组高(P<0.05);HBV-DNA低拷贝组、HBV-DNA高拷贝组外周血CD3^+、CD3^+CD4^+、CD3-CD56^+相比HBV-DNA阴性组低(P<0.05),而CD3^+CD8^+相比HBV-DNA阴性组较高(P<0.05);经Spreaman相关性分析外周血CD3^+、CD3^+CD4^+、CD3-CD56^+与HBV-DNA载量呈负相关关系(P<0.05);而CD3^+CD8^+与HBV-DNA载量呈正相关关系(P<0.05)。结论:HBV感染患者的免疫功能降低可能与病毒复制有关,且病毒复制可加重免疫功能紊乱,值得临床推广应用。 Objective:To study the relationship between T,B and NK cell subsets in peripheral blood and nucleic acid replication in patients with hepatitis B virus(HBV)infection.Methods:74 cases of HBV infection admitted to our hospital from July 2017 to April 2018 were taken as the study group.In addition,50 cases of health personnel who underwent physical examination in our hospital at the same time were recorded as the control group.The levels of T,B and NK cell subgroups in peripheral blood of the two groups were compared.According to different HBV-DNA load levels,the study group was divided into 20 patients in the HBV-DNA negative group,30 patients in the HBV-DNA low-copy group,and 24 patients in the HBV-DNA high-copy group.Peripheral blood T,B,and NK cell subgroups of the three groups were compared and analyzed for correlation.Results:The levels of CD3^+,CD3^+CD4^+and CD3-CD56^+in peripheral blood of the study group were lower than those of the control group(P<0.05),while the level of CD19+was higher than that of the control group(P<0.05).Peripheral blood CD3^+,CD3^+CD4^+and CD3-CD56^+in the HBV-DNA low-copy group and the HBV-DNA high-copy group were lower than those in the HBV-DNA negative group(P<0.05),while CD3^+CD8^+was higher than that in the HBV-DNA negative group(P<0.05).According to Spreaman correlation analysis,there was a negative correlation between peripheral blood CD3+,CD3+CD4+and CD3^-CD56^+and HBV-DNA load;CD3^+CD8^+was positively correlated with HBV-DNA load(P<0.05).Conclusion:The decrease of immune function in patients with HBV infection may be related to virus replication,and virus replication may aggravate immune dysfunction,which is worthy of clinical promotion and application.
