氯吡格雷联合阿司匹林对短暂性脑缺血发作患者血小板功能的影响

Effect of clopidogrel combined with aspirin on platelet function in patients with transient ischemic attack

目的探讨氯吡格雷联合阿司匹林对短暂性脑缺血发作患者血小板功能的影响。方法选择2016年4月至2017年6月收治的短暂性脑缺血发作患者86例,将其按随机双盲法分为两组,各43例。对照组给予阿司匹林治疗,观察组给予氯吡格雷联合阿司匹林治疗。比较两组患者临床疗效、血小板功能、凝血功能指标。结果治疗14 d后观察组总有效率为95.35%,高于对照组的79.07%,差异有统计学意义(P<0.05);治疗14 d后,两组血小板聚集率、血小板颗粒膜蛋白140(GMP-140)水平下降,部分凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(ATPP)升高,且观察组各指标优于对照组,差异有统计学意义(P<0.05)。结论氯吡格雷联合阿司匹林治疗短暂性脑缺血发作疗效佳,可有效抑制血小板聚集,改善凝血功能,值得临床推广。

Objective To investigate the effect of clopidogrel combined with aspirin on platelet function in patients with transient ischemic attack.Methods 86 patients with transient ischemic attack admitted to hospital from April 2016 to June 2017 were divided into two groups by random and double-blindness method,with 43 cases in each group.Control group were treated with aspirin,and observation group were given clopidogrel plus aspirin.And then,clinical efficacy,platelet functions and coagulation functions were compared between the two groups.Results After 14 days of treatment,the total effective rate of the observation group(95.35%)was higher than that of the control group(79.07%),difference was statistically significant(P<0.05).After 14 days of treatment,platelet aggregation rates and platelet granule membrane protein 140(GMP-140)levels decreased in both groups,but partial prothrombin time(PT),thrombin time(TT)and activated partial thromboplastin time(ATPP)increased in both groups,and the indexes of the observation group were superior to those of the control group,difference was statistically significant(P<0.05).Conclusion Clopidogrel combined with aspirin is effective in the treatment of transient ischemic attack,which can effectively inhibit platelet aggregation and improve coagulation function,and thus it is worthy of clinical promotion.
