有限内固定结合外固定支架与双侧钢板内固定在复杂性胫骨平台骨折中的对比

摘要 目的对比分析有限内固定结合外固定支架与双侧钢板内固定治疗复杂性胫骨平台骨折的效果。方法研究对象为2015年1月至2018年1月收治的92例复杂性胫骨平台骨折患者,按抽签法分为两组。对照组46例采用双侧钢板内固定治疗,研究组46例采用有限内固定结合外固定支架治疗,术后随访12个月。比较两组手术情况、膝关节功能、并发症。结果对照组手术时间、骨折愈合时间长于研究组,术中失血量多于研究组,膝关节功能总优良率低于研究组,并发症总发生率高于研究组,差异有统计学意义(P<0.05)。结论相较于双侧钢板内固定,复杂性胫骨平台骨折采用有限内固定结合外固定支架治疗创伤性更小,可降低并发症发生率,有利于促进术后膝关节功能恢复。 Objective To compare and analyze the effects of limited internal fixation combined with external fixator and bilateral plate internal fixation in the treatment of complex fracture of tibial plateau.Methods 92 patients with complex fracture of tibial plateau admitted to hospital from January 2015 to January 2018 were selected as research subjects and divided into two groups by draw lots.Control group(46 cases)were treated with bilateral plate internal fixation,while research group(46 cases)were treated with limited internal fixation combined with external fixator.And then,all patients were followed up for 12 months,and operation condition,knee joint function and complications were compared between the two groups.Results Operation time and fracture healing time in the control group were longer than those in the research group,intraoperative bleeding volume of the control group was more than that of the research group,the total excellent rate of knee joint function in the control group was lower than that in the research group,and the total incidence of complications in the control group was higher than that in the research group,difference was statistically significant(P<0.05).Conclusion Compared with bilateral plate internal fixation,limited internal fixation combined with external fixator in the treatment of complex fracture of tibial plateau has less trauma,which can decrease the incidence of complications and promote the postoperative recovery of knee joint function.
