加味五虎追风散治疗肾虚血瘀证帕金森病合并便秘的临床观察

A Clinical Observation on Modified Wuhu Zhuifeng Powder(加味五虎追风散) for Parkinson's Disease complicated with Constipation

目的:观察加味五虎追风散治疗肾虚血瘀证帕金森病合并便秘的临床疗效。方法:将80例肾虚血瘀证帕金森病合并便秘患者随机分为治疗组和对照组,每组40例,治疗组予多巴丝肼片联合加味五虎追风散治疗,对照组予多巴丝肼片联合乳果糖口服溶液治疗,连续治疗4周后观察疗效。结果:治疗组、对照组便秘疗效总有效率分别为95.00%、90.00%,两组比较,差异有统计学意义(P<0.05);治疗后两组患者中医证候较治疗前均有所改善,但治疗组优于对照组(P<0.05);两组患者治疗前后UPDRSⅡ、UPDRSⅢ积分均有所降低(P<0.05或P<0.01),治疗组患者治疗后UPDRSⅡ、UPDRSⅢ积分均低于对照组(P<0.05);治疗组患者治疗后PAC-QOL及PDQ-39量表评分降低程度优于对照组(P<0.05);停药1个月后随访,治疗组复发率(7.5%)低于对照组(25.0%)。结论:加味五虎追风散可明显改善肾虚血瘀证帕金森病合并便秘患者的便秘症状及中医证候,改善帕金森病患者的日常活动及运动症状,提高患者的生活质量,疗效确切,复发率低。

Objective: To observe the therapeutic effect of Modified Wuhu Zhuifeng power for Parkinson's disease of blood stasis and kidney deficiency complicated with constipation. Methods: Totally 80 cases of Parkinson's disease complicated with constipation were randomized divided into observation group and control group, with 40 cases in each. The observation group were given Benserazide tablets and Modified Wuhu Zhuifeng power for treatment, while the control group was treated with Benserazide tablets and Lactulose Oral Solution for treatment, the therapeutic effect was observed after 4 weeks. Results: The total effective rates of constipation in observation group and control group were 95.00% and 90.00% respectively, with significant difference (P <0.05). After treatment, the symptoms of TCM in two groups improved, but the observation group was better than the control group (P<0.05). After treatment, UPDRSⅡand UPDRSⅢ scores in both groups were lower than those before treatment (P <0.05 or P <0.01). And UPDRSⅡand UPDRSⅢ scores in the observation group were lower than those in the control group (P<0.05). After treatment, PAC-QOL and PDQ-39 scores in the observation group were lower than those in the control group (P<0.05). One month after drug withdrawal, the recurrence rate in the observation group was 7.5%, which was lower than that in the control group (25.0%). Conclusion: Modified Wuhu Zhuifeng Powder can obviously improve the constipation symptoms and TCM syndromes of Parkinson's disease, improve the daily activities and motor symptoms of Parkinson's disease patients, improve the quality of life of patients, and have definite curative effect and low recurrence rate.
