外用rhGM-CSF凝胶联合磺胺嘧啶银乳膏治疗小面积深Ⅱ度难愈创面的疗效及安全性

Efficacy and Safety of External rhGM-CSF Gel Combined with Sulfadiazine Silver Cream in Treatment of Patients with Small Area Deep Ⅱ Degree Refractory Wound

目的探讨外用重组人粒细胞巨噬细胞刺激因子(rhGM-CSF)凝胶联合磺胺嘧啶银乳膏治疗小面积深Ⅱ度难愈创面的疗效及安全性。方法选取2016年1月—2017年12月我院收治的114例小面积深Ⅱ度难愈创面患者作为研究对象,按不同治疗方法分为对照组50例和观察组64例。对照组给予磺胺嘧啶银乳膏,观察组在对照组基础上联合rhGM-CSF凝胶外用,治疗结束后记录并比较2组创面情况、临床疗效和药物不良反应等情况。结果与治疗前比较,2组创面分泌物逐渐减少,创缘反应也逐渐减轻,且治疗后不同时间点观察组创面分泌物、创缘反应优于对照组(P<0.05)。观察组创面愈合率高于对照组,创面愈合所需时间短于对照组(P<0.05);观察组临床总有效率高于对照组(P<0.05)。2组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论外用hGM-CSF凝胶联合磺胺嘧啶银乳膏可加快烧伤后患者小面积深Ⅱ度难愈创面的愈合,提高临床有效率,安全性可,值得作为临床上治疗小面积深Ⅱ度难愈创面患者治疗方案之一。

Objective To investigate efficacy and safety of external human granulocyte-macrophage colony stimulating factor (rhGM-CSF) Gel combined with Sulfadiazine Silver Cream in treatment of patients with small area deep Ⅱ degree refractory wound. Methods A total of 114 patients with small area deep Ⅱ degree refractory wound admitted during January 2016 and December 2017 were divided into control group ( n =50) and observation group ( n =64). Control group was treated with Sulfadiazine Silver Cream, while observation group was added with external rhGM-CSF Gel on the basis of treatment for control group. After treatment, wound conditions, clinical efficacy and adverse reactions were recorded and compared between two groups. Results Wound secretion and wound margin reaction after treatment were decreased gradually compared with those before treatment in two groups, and wound secretion and wound margin reaction in observation group were better than those in control group at different time points after treatment ( P <0.05). In observation group, wound healing rate was significantly higher, and wound healing time was significantly shorter than those in control group ( P <0.05). The total clinical effective rate in observation group was significantly higher than that in control group ( P <0.05). There was no significantly difference in incidence rate of adverse reactions between two groups ( P > 0.05). Conclusion External rhGM-CSF gel combined with Sulfadiazine Silver Cream may accelerate wounds healing and improve clinical efficiency rate with safety in treatment of patients with small area deep Ⅱ degree refractory wound, and therefore it is worthy of being used as one of methods in clinic.
