骨关节炎患者滑膜组织中基质金属蛋白酶3和半乳糖凝集素3的表达及相关性 被引量:12

Expression and correlation of matrix metalloproteinase-3 and galectin-3 in synovial tissue of osteoarthritis patients

摘要 背景:基质金属蛋白酶3与半乳糖凝集素3在骨关节炎的发病过程中各自发挥重要作用,两者之间可能存在某种潜在的联系,通过查阅大量文献未见报道。目的:检测骨关节炎患者滑膜中基质金属蛋白酶3和半乳糖凝集素3的表达水平,探讨其在骨关节炎发病机制中的作用和关系。方法:骨关节炎观察组取人工膝关节置换或膝关节镜治疗骨关节炎患者的关节滑膜组织,共60例;非骨关节炎对照组取创伤后行截肢术健康青年患者或急性损伤导致半月板或膝关节韧带损伤行关节镜治疗患者切除的正常膝关节滑膜组织,共20例。所有供者均对试验过程知情同意,试验方案得到石河子大学医学院第一附属医院的伦理批准。根据西安大略和麦克马斯特大学骨关节炎指数将骨关节炎观察组分为轻度、中度、重度3组,每组20例。免疫组织化学法检测骨关节炎患者滑膜组织中基质金属蛋白酶3和半乳糖凝集素3的表达,并对相关指标进行Pearson相关性分析。结果与结论:①基质金属蛋白酶3免疫组化染色结果:非骨关节炎对照组2例阳性,阳性率为10%;轻度骨关节炎观察组7例阳性,阳性率为35%;中度骨关节炎观察组13例阳性,阳性率为65%;重度骨关节炎观察组18例阳性,阳性率为90%;②半乳糖凝集素3免疫组化染色结果:非骨关节炎对照组1例阳性,阳性率为5%;轻度骨关节炎观察组6例阳性,阳性率为30%;中度骨关节炎观察组12例阳性,阳性率为60%;重度骨关节炎观察组17例阳性,阳性率为85%;③骨关节炎患者滑膜组织中基质金属蛋白酶3与半乳糖凝集素3的表达呈现显著正相关关系(r=0.926,P<0.001),其表达亦随着骨关节炎的加重而增加;④结果提示,骨关节炎组滑膜组织中基质金属蛋白酶3与半乳糖凝集素3均表现为高表达状态,在骨关节炎观察组与非骨关节炎对照组中两者的表达差异显著,共同在骨关节炎中发挥作用。基质金属蛋白酶3与半乳糖凝集素3在骨关节炎病变过程中均发挥重要的调控作用,在骨关节炎软骨退变和滑膜增生等病理变化过程中两者关系密切,这为进一步研究两者相互作用机制提供了新的思路,同时为骨关节炎的治疗及预防提供了新的理论指导和实验依据。 BACKGROUND:Matrix metalloproteinase-3 and galectin-3 play important roles in the pathological process of osteoarthritis.There may be some potential relationship between them,but has not been reported.OBJECTIVE:To detect the expression levels of matrix metalloproteinase-3 and galectin-3 in the synovial tissue of osteoarthritis,and to investigate the roles and relationship between matrix metalloproteinase-3 and galectin-3 in the pathogenesis of osteoarthritis.METHODS:Synovial tissues were obtained from the 60 patients undergoing total knee arthroplasty or knee arthroscopic treatment surgery and 20 healthy patients suffering from amputation caused by trauma or meniscus or knee ligament injuries caused by acute injury.The informed consents were obtained from all patients,and the study was approved by the Ethics Committee of First Affiliated Hospital of Medical College of Shihezi University.According to the Western Ontario and McMaster Universities Osteoarthritis Index,the osteoarthritis observation group was divided into three groups:mild,moderate,and severe groups,with 20 cases in each group.The expression levels of matrix metalloproteinase-3 and galectin-3 in the synovial tissues were detected by immunohistochemistry,and Pearson correlation analysis was conducted.RESULTS AND CONCLUSION:(1)Immunohistochemistry results showed that matrix metalloproteinase-3 was positive in 2(10%)in the non-osteoarthritis group,positive in 7(35%)samples in the mild-osteoarthritis group,positive in 13(65%)samples in the medium-osteoarthritis group and positive in 18(90%)samples in the severe-osteoarthritis group.(2)Galectin-3 was positive in 1(5%)sample in the non-osteoarthritis group,positive in 6(30%)samples in the mild-osteoarthritis group,positive in 12(60%)samples in the medium-osteoarthritis group and positive in 17(85%)samples in the severe-osteoarthritis group.(3)The expression levels of matrix metalloproteinase-3 and galectin-3 in the synovial tissues presented a positive correlation(r=0.926,P<0.001),and the levels increased with joint degeneration becoming severe.(4)These results imply that there is a significant increase in the expression levels of matrix metalloproteinase-3 and galectin-3 in the synovial tissues of osteoarthritis compared with the non-osteoarthritis group,and both exert effect in osteoarthritis.Matrix metalloproteinase-3 and galectin-3 play important roles in regulating the process of osteoarthritis and related mutually,thus providing new theoretical direction and experimental basis for targeted gene therapy of osteoarthritis.
