睡眠呼吸暂停低通气综合征合并心律失常的危险因素分析

Analysis of risk factors and polysomnography in patients with sleep apnea hypopnea syndrome and arrhythmia

目的探讨睡眠呼吸暂停低通气综合征(SAHS)合并心律失常患者危险因素及多导睡眠图分析。方法选取我院因睡眠打鼾就诊的患者197例,根据多导睡眠图检查结果将其分为单纯鼾症组27例和SAHS组170例,并将170例SAHS患者分为SAHS未合并心律失常组(n=74例)及SAHS合并心律失常组(n=96例)。统计多导睡眠图检查结果,并分析发生心律失常的影响因素。结果SAHS组LSaO2、MSaO2低于单纯鼾症组,AHI高于单纯鼾症组,鼾声时间、LAT、AAT长于单纯鼾症组(P<0.05);饮酒史、左房大小、SOD1、TNF-α、iNOS、BMI是SAHS合并心律失常发生的影响因素(P<0.05);BMI、左房大小、iNOS是预测SAHS合并心律失常发生的高危因素(P<0.05)。结论SAHS患者心律失常发生率较高,BMI、左房大小、iNOS是预测SAHS合并心律失常发生的高危因素。而多导睡眠图分析发现,低氧、通气功能障碍是其重要影响因素。

Objective To investigate the risk factors and polysomnography of patients with sleep apnea hypopnea syndrome(SAHS)complicated with arrhythmia.Methods 197 patients who were treated for sleep snoring in our hospital were randomly divided into 27 cases in the snoring group and 170 cases in the SAHS group according to the results of polysomnography.170 patients with SAHS were divided into the SAHS patients without arrhythmia group(n=74)and the SAHS complicated with arrhythmia group(n=96).The results of statistical polysomnography were analyzed and the influencing factors of arrhythmia were analyzed.Results LSaO2 and MSaO2 in the SAHS group were lower than those in the simple snoring group,AHI was higher than that in the simple snoring group,and snoring time,LAT and AAT were longer than those in the simple snoring group(P<0.05).Alcohol drinking history,left atrial size,SOD1,TNF-a,iNOS and BMI were the influencing factors of SAHS with arrhythmia(P<0.05).BMI,left atrial size,and iNOS were the predictors of SAHS with arrhythmia(P<0.05).Conclusion The incidence of arrhythmia in SAHS patients is higher.BMI,left atrial size and iNOS are high risk factors for SAHS complicated with arrhythmia.Polysomnography shows that hypoxia and ventilatory dysfunction are important factors.
