摘要
目的探讨影响大鼠心搏骤停复苏后早期(24h)存活率的可能相关因素。方法通过建立窒息法大鼠心搏骤停复苏模型,将复苏成功的33只大鼠根据24h存活情况分为两组,其中存活组20只和死亡组13只。记录并比较两组大鼠基础情况、心搏骤停后初始心律、复苏时间、复苏后电解质、氧合情况、酸碱平衡等指标。结果两组大鼠基础情况比较差异无统计学意义(P>0.05);存活组比死亡组复苏持续时间明显减少,差异有统计学意义[(83.65±39.97)s vs.(173.08±70.43)s,P<0.01]。存活组与死亡组大鼠复苏后血气分析显示,pH(7.24±0.07 vs.7.15±0.06,P<0.01)、总二氧化碳(TCO2)水平[(20.44±2.68)mmol/L vs.(17.99±2.61)mmol/L,P=0.02]存活组明显高于死亡组;而乳酸(Lac)值[(3.91±1.67)mmol/L vs.(5.43±1.09)mmol/L,P=0.01]存活组明显低于死亡组。结论复苏持续时间、高乳酸血症及代谢性酸中毒是大鼠早期死亡的重要影响因素。
Objective To investigate the related factors influencing the early survival rate(24 h)during post-resuscitation of cardiac arrest in rats.Methods Establish the asphyxial cardiac arrest model and 33 rats successfully resuscitated,and there were 20 rats in the survival group and 13 rats in the death group after 24 hours.The test compared the indexes between two groups in baseline data,rhythm of initial recovery,resuscitation duration and electrolyte,oxygenation,acid-base balance during post-resuscitation.Results There was no significant difference on baseline data between the two groups(P>0.05).The duration of resuscitation in the survival group was significantly shorter than that in the death group [(83.65±39.97)s vs.(173.08±70.43)s],and the difference was statistically significant(P<0.01);Comparison of blood gas analysis showed pH value(7.24±0.07 vs.7.15±0.06,P<0.01)and total carbon dioxide[TCO2,(20.44±2.68)mmol/L vs.(17.99±2.61)mmol/L,P=0.02]in the survival group were significantly higher than that in the death group.But the lactic acid[Lac,(3.91±1.67)mmol/L vs.(5.43±1.09)mmol/L,P=0.01]in the survival group was statistically significantly decreased than the death group.Conclusion The duration of resuscitation,hyperlactic acidosis and metabolic acidosis are important factors that affect the survival rate of rats in post-resuscitation.
