MSCT评估不同病理类型儿童肝母细胞瘤的价值 被引量:2

The value of MSCT in diagnosing hepatoblastoma of different pathological types in children

摘要 目的分析不同病理类型的儿童肝母细胞瘤(hepatoblastoma, HB)的MSCT表现,提高对该病的认识。方法回顾性分析31例经手术或穿刺活检病理证实的HB患者的MSCT资料。按照HB病理结果将是否为胎儿型分为两组。采用Fisher精确检验评估两组HBCT征象的差异性。采用两独立样本的t检验,比较两组肿瘤最大径以及两组病灶各期扫描CT值的差异性。结果胎儿型HB较其它病理类型HB坏死、转移少见,有统计学意义(P<0.05)。两组肿瘤最大径比较,差异无统计学意义(P>0.05)。平衡期CT值比值,两组比较差异有统计学意义(P<0.05)。结论胎儿型HB的MSCT形态学及增强表现与其它病理类型HB不同,MSCT可初步诊断胎儿型HB。 Objective To analyze the multi-slice spiral computed tomography (MSCT) imaging features of hepatoblastoma (HB) in children with different pathological types,and in order to improve the ability to recognize this disease. Methods The MSCT data of 31 patients with HB confirmed by operation or biopsy pathology were retrospectively evaluated. Theses imaging data were divided into two groups according to the pathological result of HB and whether it was a fatal type or not.The difference in CT signs of HB between the two groups were tested by Fisher exact test. The difference in the maximum diameter of tumor and in CT values at different phase between the two groups were compared by two independent-samples t test. Results The sign of necrosis and metastasis in fetal type HB was less than in other pathological types of HB, and the difference was statistically significant ( P <0.05). The difference of the maximum diameter of tumor between the two groups was not statistically significant ( P >0.05). The ratio of the CT value in equilibrium phase, the differences of which between the two groups was statistically significant ( P <0.05). Conclusion The MSCT morphology and enhancement of the fetal type HB were different from other pathological types HB's, and the MSCT could give an preliminary diagnosis of fetal HB.
