腹腔镜卵巢囊肿剥除术对患者术后卵巢功能的影响

Influence of laparoscopic ovarian cystectomy on postoperative ovarian function of patients

目的 探讨腹腔镜卵巢囊肿剥除术对患者术后卵巢功能的影响。方法 将80例卵巢囊肿患者随机分为2组,开腹组患者给予开腹卵巢囊肿剥除术,腹腔镜组患者则采用腹腔镜卵巢囊肿剥除术。比较2组卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、抗苗勒氏管激素(AMH)、抑制素B(IHHB)、窦卵泡计数(AFC)、血管搏动指数(PI)、血管阻力指数(RI)、收缩期峰值血流速度(PSV)、CD3^+、CD4^+、CD8^+等指标。结果 腹腔镜组患者术后1、6、12个月的FSH与LH水平均低于开腹组,E2、AMH与IHHB水平均高于开腹组,差异有统计学意义(P<0.05);2组患者治疗前后AFC水平无显著差异(P>0.05)。腹腔镜组患者术后1、6、12个月的PI与RI水平均低于开腹组,PSV水平高于开腹组,差异有统计学意义(P<0.05);2组患者治疗前后CD3^+、CD4^+、CD8^+水平无显著差异(P>0.05)。结论 相比于传统开腹手术,腹腔镜卵巢囊肿剥除术对患者术后卵巢功能的恢复效果更佳。

Objective To investigate the effect of laparoscopic ovarian cystectomy on postoperative ovarian function of patients. Methods Totally 80 patients with ovarian cysts were randomly divided into two groups. Patients in the open surgery group were treated with open ovarian cystectomy, while patients in the laparoscope group were treated with laparoscopic ovarian cystectomy. The follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E 2), anti-Mullerian hormone (AMH), inhibin B (IHHB), sinus follicle count (AFC), vascular pulsation index (PI), vascular resistance index (RI), peak systolic flow velocity (PSV), CD3^+, CD4^+ and CD8^+ were compared between the two groups. Results The levels of FSH and LH in laparoscope group were significantly lower than those in open surgery group at 1, 6 and 12 months after operation, and the levels of E 2, AMH and IHHB were significantly higher than those in open surgery group ( P <0.05). There was no significant difference in AFC level between the two groups before and after treatment ( P > 0.05). The PI and RI values in laparoscope group were significantly lower than those in open surgery group at 1, 6 and 12 months after operation, and the PSV value was significantly higher than that in open surgery group ( P <0.05). There was no significant difference in CD3^+, CD4^+, CD8^+ levels between the two groups before and after treatment ( P >0.05). Conclusion Compared with open surgery, laparoscopic ovarian cystectomy has better recovery effect on postoperative ovarian function.
