结肠癌CT诊断临床价值研究及展望

Clinical value and Prospect of CT diagnosis of Colon Cancer

目的:学习总结结肠癌临床病例的CT表现,探讨多层螺旋CT对结肠癌诊断的临床价值。方法:收集笔者所在医院89例经手术病理确诊为结肠癌的病例,回顾分析CT平扫及增强表现,结合多平面重建技术(MPR),观察病变部位、大小、形态、边界以及周围侵犯和腹腔远处转移情况,总结结肠癌术前影像诊断及分期经验,评估术前CT诊断价值。结果:左半结肠34例,右半结肠25例,乙状结肠30例;大体病理类型呈息肉型或肿块型42例,溃疡型15例,浸润型或狭窄型32例。CT检测肿瘤部位与手术结果完全一致,术前CT分期与术后临床分期总符合率达89.8%。结论:CT对结肠癌检查准确高,同时亦能评估肿瘤周围侵犯情况及远处转移情况,对临床分期有重要指导作用。

Objective: To summarize the CT features of colon cancer and discuss the clinical value of MSCT in the diagnosis of colon cancer.Methods: 89 cases of colon cancer were collected from our hospital.Retrospective analysis of CT features,combined with multiplanar reconstruction (MPR),to observe the lesion location, size,shape,boundary,peritoneal and abdominal metastasis.To further summarize the experience of imaging diagnosis and staging of colon cancer,and to evaluate the value of preoperative CT diagnosis.Results: There were34 cases of left colon,25 cases of right colon and30 cases of sigmoid colon.The gross specimens were polypoid or mass type in 42 cases,ulcerative type in15 cases,invasive or narrow type in32 cases.The CT examinations of the locations of the tumors were completely consistent with the results of surgery.The total coincidence rate between preoperative CT staging and postoperative clinical staging was 89.8%.Conclusion: CT has high accuracy in the examination of colon cancer.It can also evaluate the invasion and distant metastasis of colon cancer.It plays an important guiding role in clinical staging.
