摘要
目的探讨新疆地区单中心主动脉夹层发病的时间特点,月相周期与主动脉夹层发病的关系,提高可预测性和预防性。方法收集2009年1月-2015年8月在新疆医科大学第一附属医院诊断为主动脉夹层并接受治疗的734例患者的临床资料,详细记录患者的发病时间,确定每位患者发病时所处的月相时期,并分析每日、每周、每月、每个季节及不同月相时期主动脉夹层的发病情况。结果主动脉夹层患者发病平均年龄为(50.43±11.93)岁,男性(83.7%)较女性(16.3%)多发,高血压患者患B型夹层的危险性是患A型的1.875倍。A型患者不同时间段发病人数分布差异有统计学意义(P<0.05),17∶00-20∶00发病人数最多(33.3%)。B型夹层患者不同季节发病人数分布差异有统计学意义(P<0.05),冬季发病人数最多(30.3%)。在月相分布上,夹层患者多发病于上弦月及满月期,差异有统计学意义(P<0.05)。结论主动脉夹层的发病表现出季节性模式及月相模式,在高发的季节及月相期间做出保护性措施对预防主动脉夹层的发病可能是有益的。
Objective To explore the time characteristics of the onset of single-center aortic dissection in Xinjiang and the relationship between the lunar phase cycle and the onset of aortic dissection, and to improve predictability and prevention. Methods The clinical data of 734 patients with aortic dissection diagnosed and treated in the First Affiliated Hospital of Xinjiang Medical University from January 2009 to August 2015 were collected. The onset time of aortic dissection was recorded in detail, and the monthly phase of each patient was determined. The incidence of aortic dissection was analyzed daily, weekly, monthly, seasonal and different monthly phases. Results The average age of patients with aortic dissection was(50.43±11.93) years old. The proportion of males(83.7%) was higher than that of females(16.3%). The risk of type B dissection in patients with hypertension was 1.875 times higher than that in patients with type A. There was the significant difference in the distribution of the number of patients with type A in different time periods(P<0.05). The proportion(33.3%) of patients from 17∶00 to 20∶00 was the highest. There was significant difference in the incidence of type B interlayer in different seasons(P<0.05), and the incidence of type B interlayer was the highest in winter(30.3%). On the distribution of the lunar phase, the incidence of dissection was higher in the first quarter and the full moon(P<0.05). Conclusion The incidence of aortic dissection shows a seasonal pattern and a phase pattern. It may be beneficial to prevent protective aortic dissection in the high-intensity season and phase.
