保乳术联合前哨淋巴结活检术与改良根治术治疗早期三阴性乳腺癌的效果比较

Comparison of effects of breast-conserving surgery combined with sentinel lymph node biopsy and modified radical mastectomy in treatment of patients with early triple negative breast cancer

目的:比较保乳术联合前哨淋巴结活检术与改良根治术治疗早期三阴性乳腺癌的效果。方法:回顾性分析2015年1月至2016年12月的62例早期三阴性乳腺癌患者的临床资料,根据治疗方法不同分为对照组及观察组各31例。对照组患者行改良根治术治疗,观察组采用保乳术联合前哨淋巴结活检术治疗。比较两组患者手术情况和术后恢复情况、乳房美容满意率、手术后1年治疗效果、术后3年复发率及并发症发生情况。结果:观察组患者乳房美容满意率为96.77%,明显高于对照组的67.74%,差异具有统计学意义(P<0.05);观察组患者手术时间、住院时间及胸壁引流管留置时间明显短于对照组,总引流量、术中出血量明显少于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为3.23%,明显低于对照组的19.35%,差异具有统计学意义(P<0.05);两组术后1年治疗总有效率和术后3年复发率比较,差异均无统计学意义(P>0.05)。结论:保乳术联合前哨淋巴结活检术治疗早期三阴性乳腺癌的效果优于改良根治术治疗。

Objective:To compare effects of breast-conserving surgery combined with sentinel lymph node biopsy and modified radical mastectomy in treatment of patients with early triple negative breast cancer.Methods:The clinical data of 62 patients with early triple negative breast cancer from January 2015 to December 2016 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,31 cases in each.The control group was treated with the modified radical mastectomy,while the observation group was treated with the breast-conserving surgery combined with sentinel lymph node biopsy.The operation and postoperative recovery,breast cosmetic satisfaction rate,1-year treatment effects,3-year recurrence rate,and complications were compared between the two groups.Results:The breast cosmetic satisfaction rate in the observation group was 96.77%,which was significantly higher than that in the control group(67.74%),and the difference was statistically significant(P<0.05).The operation time,hospitalization time,and indwelling time of the chest wall drainage tube were significantly shorter in the observation group than in the control group;the total drainage volume and intraoperative blood loss were significantly less than those in the control group;and the differences were statistically significant(P<0.05).The complication rate in the observation group was 3.23%,which was significantly lower than that in the control group(19.35%),and the difference was statistically significant(P<0.05).There were no significant differences in the 1-year total effective rate and 3-year recurrence rate between the two groups(P>0.05).Conclusions:The breastconserving surgery combined with sentinel lymph node biopsy in the treatment of the patients with early triple negative breast cancer is superior to the modified radical mastectomy.
