AVAPS无创机械通气治疗老年急性心力衰竭合并低氧血症的临床研究 被引量:26

摘要 目的探讨平均容量保证压力支持(AVAPS)无创机械通气治疗老年急性心力衰竭合并低氧血症的临床疗效。方法将40例老年急性心力衰竭合并低氧血症的病人随机分为治疗组和对照组,每组20例。两组均给予常规药物+无创机械通气治疗。治疗组给予AVAPS模式,对照组给予双水平气道正压(BiPAP)模式。观察两组病人治疗前后血气分析、N末端脑钠肽前体(NT-proBNP)、无创机械通气时间、不良反应等。结果两组治疗后动脉血氧分压较治疗前升高,动脉血二氧化碳分压、NT-proBNP均较治疗前明显下降,差异均有统计学意义(P<0.05);治疗组呼吸机参数人工调整次数及呼吸机带机时间均小于对照组,两组比较有统计学意义(P<0.05)。结论 AVAPS无创机械通气能快速纠正低氧血症,减少无创机械通气时间及呼吸机参数人工调整次数,可用于老年急性心力衰竭合并低氧血症病人的治疗。
