摘要
目的探讨MIPPO技术配合关节镜微创手术对低能量胫骨平台骨折患者术后康复及关节活动度的影响。方法将86例低能量胫骨平台骨折患者根据手术方法不同分为对照组43例与观察组43例,对照组采用常规切开复位内固定术治疗,观察组给予MIPPO技术配合关节镜微创手术治疗,对两组患者术后康复情况、关节活动度、膝关节功能进行观察。结果观察组手术切口长度、术后下床活动时间、住院时间、骨折愈合时间与对照组比较,差异有统计学意义(P<0.05);观察组术后伸膝最大角度(-0.32±0.11)°和屈膝最大角度(129.54±7.68)°,均明显大于对照组(-0.64±0.18)°、(110.86±5.97)°,差异有统计学意义(t=9.947,12.592,P<0.05);两组术前的Lysholm膝关节功能评分无明显差异(P>0.05),观察组术后Lysholm膝关节功能评分(92.37±2.64)分,明显优于对照组(81.59±2.71)分,差异有统计学意义(t=18.684,P<0.05)。结论 MIPPO技术配合关节镜微创手术治疗低能量胫骨平台骨折创伤小,利于术后康复,也能改善患者膝关节功能及活动能力,值得推广。
Objective To investigate the effect of MIPPO technique combined with arthroscopic minimally invasive surgery on postoperative rehabilitation and joint mobility in patients with low energy tibial plateau fractures.Methods 86 patients with low-energy tibial plateau fractures were divided into 43 cases in the control group and 43 cases in the observation group.The control group was treated with conventional open reduction and internal fixation.The observation group was given MIPPO technique combined with arthroscopic minimally invasive surgery.The postoperative rehabilitation,joint mobility,and knee function were observed in the two groups.Results The length of the incision in the observation group,the time of postoperative outpatient activity,the length of hospital stay,and the time of fracture healing were significantly different from those in the control group,the difference was statistically significant(P<0.05).The maximum angle of knee extension(-0.32±0.11)°and knee flexion maximum angle in the observation group(129.54±7.68)°was significantly greater than that of the control group(-0.64±0.18)°,(110.86±5.97)°,the difference was statistically significant(t=9.947,12.592,P<0.05).The preoperative Lysholm knee function scores in the two groups had no significant difference(P>0.05).The Lysholm knee function score(92.37±2.64)pornts in the observation group was significantly better than that in the control group(81.59±2.71)pornts,the difference was statistically significant(t=18.684,P<0.05).Conclusion MIPPO technique combined with arthroscopic minimally invasive surgery for the treatment of low-energy tibial plateau fractures is less invasive,which is conducive to postoperative rehabilitation and can improve knee joint function and activity.It is worthy of promotion.
