肺部真菌感染33例临床分析

Clinical Observation of 33 Cases of Pulmonary Fungal Infection in Respiratory Medicine

目的 观察分析该院肺部真菌感染患者的临床表现,分析具体的诊断治疗方法及其效果。方法 选取2016年2月—2018年2月期间在该院接受治疗的肺部真菌感染患者33例作为研究对象,分析所有患者的临床资料,具体包括患者个人资料、基础疾病情况、感染的真菌类型以及采用的治疗措施及其临床效果。结果 33例肺部真菌感染患者中,白色假丝酵母菌感染20例,占比60.1%,曲霉菌感染5例,占比15.2%,克柔假丝酵母菌、热带假丝酵母菌、光滑念珠菌各2例,各占比6.1%,肺孢子菌感染2例,占比6.1%。经过检验,发现33例肺部真菌感染患者中存在免疫缺陷或者机体免疫力低下共27例,占比81.8%。结论 肺部真菌感染患者临床上以白色假丝酵母菌、曲霉菌感染为主,此外还会出现克柔假丝酵母菌、热带假丝酵母菌、光滑念珠菌、肺孢子菌感染等;发生肺部真菌感染的患者大部分存在免疫缺陷或者免疫力低下;针对患者感染类型实施对应治疗措施以后,治疗效果与患者自身基础疾病以及免疫情况等存在紧密联系。

Objective To observe and analyze the clinical manifestations of patients with pulmonary fungal infection in our hospital, and to analyze the specific diagnosis and treatment methods and their effects. Methods Thirty-three patients with pulmonary fungal infection who were treated in our hospital from February 2016 to February 2018 were enrolled. The clinical data of all patients were analyzed, including patient profile, underlying disease, and infected fungi, type and treatment measures and their clinical effects. Results Among 33 patients with pulmonary fungal infection, 20 cases of Candida albicans infection, accounting for 60.1%, and 5 cases of Aspergillus infection, accounting for 15.2%. There were 2 cases of Candida utilis, 2 cases of Candida tropicalis, and 2 cases of Candida glabrata, each accounting for 6.1%, and there were 2 cases of Pneumocystis infection, accounting for 6.1%. After testing, it was found that there were 27 cases of immunodeficiency or low immunity in 33 cases of pulmonary fungal infection, accounting for 81.8%. Conclusion Patients with pulmonary fungal infections are mainly infected with Candida albicans and Aspergillus, in addition to Candida krusei, Candida tropicalis, Candida glabrata, Pneumocystis infection, etc. Most patients with fungal infections have immunodeficiency or low immunity. After the corresponding treatment measures applied to the type of infection, the treatment effect is closely related to the patient's own underlying diseases and immune status.
