二维斑点追踪技术对主动脉瓣狭窄患者瓣膜置换前后左心室收缩功能的评价作用研究

摘要 目的探究二维斑点追踪技术(STI)对主动脉瓣狭窄患者瓣膜置换前后左心室收缩功能的评价作用。方法选择我院2017年1月~12月收治的主动脉瓣狭窄患者80例设为观察组,选择同期健康志愿者80例设为对照组,两组患者均给予STI。探讨STI对左心室收缩功能的评价作用。结果在左心室后壁厚度(LVPWT)、室间隔厚度(IVS)上,观察组显著比对照组高(P<0.01);观察组左室纵向应变(GLS)和应变率显著比对照组低(P<0.01)。两组在左心室收缩末期内径(LVS)、舒张末期内径(LVD)及左室射血分数(LVEF)上比较,无显著性差异(P>0.05)。观察组术后IVS、LVPWT较术前变薄(P<0.05),GLS和应变率较术前明显增加(P<0.01);而LVS、LVD及LVEF术后虽有改善,但与术前比较,无显著性差异(P>0.05)。观察组术后IVS、LVPWT持续变薄,但仍较对照组厚;GLS和应变率虽有改善,但仍低于对照组(P<0.05);而LVS、LVD及LVEF与对照组比较,无显著性差异(P>0.05)。结论将STI应用于主动脉瓣狭窄患者,能准确反映心肌的运动,且可评价心肌节段性及整体性收缩功能,此方法值得推广应用。
