43例慢性泪囊炎的鼻内镜下鼻腔泪囊造口术的临床分析

Application of endoscopic transnasal dacryocystorhinostomy for chronic dacryocystitis:a clinical analysis of 43 cases

目的分析开展鼻内镜下鼻腔泪囊造口术治疗慢性泪囊炎的手术方式和治疗效果。方法对43例慢性泪囊炎(其中7例伴有泪囊瘘管)患者施行鼻内镜下鼻腔泪囊造口术。结果38例患者溢泪消失,泪道冲洗通畅,泪囊造口处完全上皮化,呈圆孔型开放状态;3例溢泪症状减轻,泪道冲洗通畅,造口处呈裂隙状或针尖样大小;2例无效,溢泪症状未消失,泪道冲洗不通畅,造口处闭锁。总有效率为95.34%。结论鼻内镜下鼻腔泪囊造口术操作简单、出血少、损伤小、恢复快,符合现代外科手术微创和美容要求。对治疗慢性泪囊炎效果良好。

Objective To evaluate the surgical methods and therapeutic effects of the endoscopic transnasal dacryocystorhinostomy for chronic dacryocystitis. Methods 43 cases of chronic dacryocystitis(including 7 cases with lacrimal sac fistula)were treated with endoscopic transnasal dacryocystorhinostomy. Results The epiphora disappeared,lacrimal passage unobstructed,and lacrimal sac stoma like a hole,which completely epithelialized,in 38 cases.And there were 3 cases with the epiphora alleviated,lacrimal passage unobstructed,and the lacrimal sac stoma like slit or needlepoint;Meanwhile,2 cases remained ineffective.The total efficiency of the treatment was 95.34%. Conclusion Endoscopic transnasal dacryocystorhinostomy has lots of advantages,such as simple operation,less bleeding,injury and quick recovery,and meets the requirements of minimally invasive and cosmetic surgery.It is more effective in the treatment of chronic dacryocystitis.
