以空洞或囊腔为主的肺内淋巴瘤的CT表现

CT Features of Intrapulmonary Lymphoma with Cavities or Cysts

目的探讨伴发空洞或囊腔的肺内淋巴瘤的CT表现,提高对本病的认识和诊断。资料与方法收集经病理证实为肺内淋巴瘤且CT表现以空洞或囊腔为主的6例患者的临床、CT及病理学资料,并进行分析。结果6例患者中,CT表现以空洞为主的肺内淋巴瘤5例,均表现为双肺随机分布,多发厚壁空洞,可伴多发实性结节、实变、纵隔淋巴结肿大;CT表现以囊腔为主的肺内淋巴瘤1例,表现为类圆形薄壁透亮区,张力较大,其内多发分隔,囊壁不规则,周围可见磨玻璃密度影。穿刺活检证实空洞为主的病变中,3例为继发性霍奇金淋巴瘤,1例为原发性霍奇金淋巴瘤,1例为灰区淋巴瘤,1例病理学可见肿瘤坏死或继发感染改变;囊腔为主的病变为黏膜相关淋巴组织淋巴瘤。结论肺内淋巴瘤CT表现为双肺多发厚壁空洞或囊腔,伴实变、结节、肺门及纵隔多发淋巴结肿大。

Purpose To explore the CT imaging features of intrapulmonary lymphoma associated with cavities or cysts, and to improve the understanding and diagnosis of it. Materials and Methods The clinical, CT imaging and pathological data of 6 patients who were pathologically confirmed as intrapulmonary lymphoma and whose CT features were mainly cavities or cysts were collected and analyzed. Results For six patients, five intrapulmonary lymphoma cases with cavities on CT features were collected, and they all showed random distribution of bilateral lungs, multiple thick-wall cavities, and multiple solid nodules, consolidation and mediastinal lymph node enlargement. One intrapulmonary lymphoma case with cysts on CT features was collected, and it showed a quasi-circular thin-wall transparent area with large tension, multiple inside septum, irregular cyst wall and ground glass density shadow around it. Among the lesions with cavities confirmed by biopsy, there were three cases of secondary Hodgkin's lymphoma, 1 case of primary Hodgkin's lymphoma, 1 case of gray-area lymphoma, and 1 case of pathologically observed tumor necrosis or secondary infection. In addition, the cystic predominant lesion was mucosa-associated lymphoid tissue lymphoma. Conclusion The CT features of intrapulmonary lymphoma could be multiple thick-wall cavities or cysts in bilateral lungs with consolidation, nodules, hilar and multiple mediastinal lymph node enlargement.
