加味黄连膏联合肛管直肠微波仪治疗肛窦炎的疗效观察

Therapeutic effects of Jiawei Huanglian ointment combined with anal rectal microwave on anal sinusitis

目的观察加味黄连膏联合肛管直肠微波仪治疗肛窦炎患者的临床效果。方法将60例肛窦炎患者随机分为加味黄连膏治疗组(n=20)、马应龙膏治疗组(n=20)及对照组(n=20)。加味黄连膏组、马应龙组及对照组分别采用加味黄连膏、马应龙膏、医用凡士林药膏纳入肛管直肠,同时予直肠微波仪行肛管直肠微波理疗20min。各组均隔天行一次治疗,共7次,2周后结束。观察各组患者治疗前后的临床症状、体征及疗效、恢复情况等相关指标,并对比分析。结果各组患者经治疗后,加味组总有效率为90%,马应龙组总有效率为55%,对照组总有效率为20%,各组疗效比较,差异有统计学意义(P<0.01)。加味组及马应龙组治疗效果优于对照组(P<0.05);同时,加味组比马应龙组治疗效果较好(P<0.05)。各组治疗前症状体征总积分比较,差异无统计学意义(P>0.05);各组治疗后症状总积分比较,差异有统计学意义(P<0.01);与对照组比较,加味组及马应龙组症状总积分较好(P<0.05);同时,加味组对比马应龙组症状总积分较好(P<0.05)。各组复发情况比较,差异有统计学意义(P<0.01);加味组较对照组复发率低(P<0.05),加味组与马应龙组复发率比较无统计学差异(P>0.05),马应龙组与对照组复发率比较无统计学差异(P>0.05)。各组患者均未发生不良反应。结论加味黄连膏联合肛管直肠微波仪治疗肛窦炎疗效好、复发少。

Objective To examine the clinical effects of Jiawei Huanglian ointment combined with anal rectal microwave on anal sinusitis. Methods Sixty patients with anal sinusitis were recruited and randomly divided into three groups of 20 to be treated respectively with Jiawei Huanglian ointment, MaYinglong Musk Hemorrhoids Ointment and Vaseline(control group). The two ointments and Vaseline were applied into the anus and rectum, followed by 20 minutes of anal rectal microwave therapy. Each group was treated every other day for 7 times during two weeks. Clinical symptoms, signs, effects, recovery and other related indicators of each group were observed and compared. Results Response rate was 90% for Jiawei Huanglian, 55% for MaYinglong, and 20% for the controls. The inter-group differences were statistically significant(P<0.01). Outcomes of Jiawei group and Mayinglong group were better than that of the controls(P<0.05). Jiawei outperformed MaYinglong(P<0.05). Differences in assessment of symptoms and signs, which were absent before treatment(P>0.05), were observed after treatment(P<0.01). Compared with the control group, the assessment of symptoms and signs of Jiawei group and Ma Yinglong group produced better(P<0.05). Assessment of symptoms and signs of Jiawei group were better than those of Ma Yinglong group(P<0.05). There were significant differences in the relapse rate of each group(P<0.01). Recurrence rate was lower in Jiawei group than in control group(P<0.05). There was no significant difference between Jiawei group and Ma Yinglong group in relapse rate(P>0.05). There was no statistical difference in the relapse rate between Ma Yinglong group and the control group(P>0.05). No adverse reaction was observed in any of three groups of patients. Conclusion Jiawei Huanglian ointment combined with anorectal microwave can produce effective treatment for anal sinusitis with less recurrence.
