摘要
目的探讨集束干预策略在PICU对CVC导管相关性留置中心静脉导管的应用及对患者炎症因子的影响。方法选取我院2015年3月~2016年12月间收治的留置中心静脉导管(central venous catheter,CVC)患者150例,依据随机数字方法分为对照组与研究组。对照组均使用常规手段对留置中心静脉导管的患者进行护理,研究组的患者则应用集束干预策略对留置中心静脉导管患者进行有效护理,记录研究期间两组患者血清炎症因子表达情况及护理结果情况,并对其数据进行分析。结果研究组血清TNF-α、IL-6、IL-1β和IL-8表达水平显著低于对照组(P<0.05);研究组患者的留置时间及住院时间显著短于对照组(P<0.05),研究组患者的CRBSI发生时间显著长于对照组(P<0.05),研究组患者的感染率显著低于对照组(P<0.05)。结论临床对留置中心静脉导管患者应用集束干预策略进行干预,能够有效降低患者导管相关性血行感染的发生率,有利于防止患者病情的恶化,对降低炎症因子表达水平极为有利,有效缩短了置管时间与住院时间,可以对其进行推广应用。
Objective To investigate the application of cluster intervention strategy in the CVC catheter-related indwelling central venous catheter PICU and its effect on inflammatory factors of patients. Methods A total of 150 patients with central venous catheter(CVC) admitted in our hospital from March 2015 to December 2016 were enrolled. The patients were divided into the control group and the study group according to the random number method. In the control group, patients with indwelling central venous catheters were treated with conventional methods. Patients in the study group were treated with cluster intervention strategy to effectively nurse patients with indwelling central venous catheters. The expression of serum inflammatory factors and nursing results during the study period were recorded. And the data were analyzed. Results The levels of serum TNF-α, IL-6, IL-1β and IL-8 in the study group were significantly lower than those in the control group(P<0.05). The retention time and hospitalization time of the study group were significantly shorter than those of the control group(P<0.05). The time of CRBSI in the study group was significantly longer than that in the control group(P<0.05). The infection rate of the study group was significantly lower than that of the control group (P<0.05). Conclusion Clinical intervention of intensive central venous catheter patients with cluster intervention strategy can effectively reduce the incidence of catheter-related blood stream infections, help prevent the deterioration of patients' condition, and greatly reduce the expression level of inflammatory factors, effectively shorten the catheterization. It can be promoted and applied.
