安神足浴方配合舒眠中药枕治疗终末期肾脏病血液透析患者失眠的临床观察

Clinical observation on treatment of insomnia in hemodialysis patients with end stage renal disease by Anshen foot bath prescription and Shumian Chinese medicine pillow

目的观察安神足浴方配合舒眠中药枕治疗终末期肾脏病血液透析患者失眠的临床疗效。方法选取2015年6月~2017年8月在我院治疗的维持性血液透析且伴有失眠症状的患者60例,将60例有失眠症状的患者随机分为对照组和治疗组,其中对照组予以基础治疗,治疗组在此基础上加用安神足浴方配合舒眠中药枕治疗,共治疗4周,对比患者匹兹堡睡眠质量指数(pittsburgh sleep quality index,PSQI)、临床疗效及血清磷、血清钙、血清甲状旁腺素、血红蛋白等指标。结果治疗前两组患者各维度得分和PSQI总分比较,差异均无统计学意义(P>0.05);治疗后治疗组睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍、PSQI总分均较治疗前有所下降,且比对照组下降情况更显著(P<0.05),两组催眠药物差异无统计学意义(P>0.05)。结论安神足浴方配合舒眠中药枕能明显改善血液透析患者的失眠症状,提高生存质量,在临床值得推广运用。

Objective To observe the clinical effect of Anshen foot bath prescription and Shumian Chinese medicine pillow in treatment of insomnia in hemodialysis patients with end stage renal disease. Methods 60 patients with insomnia and hemodialysis due to end stage renal disease from June, 2015 to August 2017 were selected and randomly divided into the control group and the treatment group. Patients in the control were given basic treatment, while those in the treatment group were additionally given Anshen foot bath prescription and Shumian Chinese medicine pillow for 4 weeks. The Pittsburgh Sleep Quality Index(PSQI), clinical efficacy, and serum phosphorus, serum calcium, serum parathyroid hormone, and hemoglobin were compared. Results There were no significant differences in the scores of various dimensions between the two groups before treatment(P>0.05). The quality of sleep, sleep latency, sleep time, sleep efficiency, sleep disturbance, daytime dysfunction, and total score of PSQI were lower in the treatment group after treatment than those before treatment, and the decreases were more significant than those in the control group(P<0.05). There was no significant difference in hypnotic drugs between two groups(P>0.05). Conclusion Anshen foot bath combined with Shumian traditional Chinese medicine pillow can significantly improve the symptoms of insomnia in hemodialysis patients and improve the quality of life. It is worthy of popularization in clinical practice.
