基于JCI标准的疼痛管理机制的建立与效果研究

目的探讨护理实践中疼痛的有效性管理,为医院建立疼痛管理机制提供依据。方法采取随机抽查某三级甲等医院护理病历记录的方式,抽取2017年9—12月32个临床科室960份病历记录,以及对护理人员进行疼痛体系培训后的2018年1—4月份的960份病历,对比疼痛评估的正确性、疼痛管理质量。结果疼痛管理体系建立并对护理人员培训后,全院首次疼痛评估正确率及疼痛分级再次评估率分别为87.71%、91.67%;评估工具使用的正确性为96.15%,与疼痛管理体系建立前具有差异性(P <0.05);疼痛处理后再评估率与结合患者疼痛期望制定护理计划率较低,分别为89.79%与91.35%。结论疼痛对患者的身心造成不利影响,建立疼痛管理体系,使护士能够掌握疼痛管理相关知识进行正确的疼痛评估及采取及时有效措施,缓解和减轻患者的疼痛,帮助患者控制疼痛。

Objective To explore the effective management of pain in nursing practice, and to provide basis for hospital establishment of pain management mechanism. Methods By random sampling of nursing records, 960 medical records were collected from 32 clinical departments from September to December 2017. And the 960 medical records from January to April of 2018, after training the pain system for nursing staff. The accuracy of pain assessment and quality of pain management was compared. Results After the establishment of pain management system and the training of nurses, the correct rate of the first pain assessment and the re-evaluation rate of pain grading were 87.71%, 91.67% and 96.15% respectively. It was different from the establishment of pain management system (P < 0.05).The rate of reassessment after pain treatment and nursing plan formulation combined with pain expectation were lower, 89.79% and 91.35% respectively. Conclusion Pain has adverse effects on patients'physical and mental health. The establishment of pain management system enables nurses to grasp the knowledge of pain management and make correct pain assessment and take timely and effective measures to alleviate and alleviate patients' pain and help patients control pain.
