喜炎平注射液联合还原型谷胱甘肽治疗重症烧伤后急性肝功能损伤临床研究

Clinical study of Xiyanping injection combined with reduced glutathione in the treatment of acute liver function injury after severe burns

目的探讨喜炎平注射液联合还原型谷胱甘肽治疗重症烧伤急性肝功能损伤患者的临床疗效。方法将50例重度烧伤患者分为常规组和观察组,每组25例,均给予防治低血容量及休克、维持电解质平衡、合理选用抗生素、促进烧伤创面愈合等常规治疗,常规组在常规治疗基础上给予还原型谷胱甘肽1 200 mg,每日静脉滴注1次。观察组在常规组治疗基础上,静脉滴注喜炎平注射液250 mg,每日1次。两组患者均治疗3周。测定两组患者治疗前后血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、间接胆红素(IBIL)、C反应蛋白(CRP)、降钙素原(PCT)、肿瘤转化因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及脂多糖(LPS)含量。结果与常规组比较,观察组临床治疗总有效率提高,差异有统计学意义(P<0.05)。治疗后,两组患者血清ALT、AST、TBIL、IBIL、CRP、PCT、LPS、TNF-α、IL-6及IL-8含量均降低,观察组较常规组降低更为显著,差异均有统计学意义(P<0.05)。结论喜炎平注射液联合还原型谷胱甘肽治疗重症烧伤后肝功能急性损伤患者安全、有效,可显著降低炎症水平。

Objective To investigate the clinical effect of Xiyanping injection combined with reduced glutathione on severe burn patients with acute liver function injury.Methods All 50 cases of severe burn patients were randomly divided into routine group and observation group,25 cases in each group,were performed the prevention and treatment of low blood volume and shock,to maintain electrolyte balance,reasonable choice of antibiotics,promote healing of burn wounds,such as conventional treatments.The patients of the conventional group were on the basis of conventional treatment given reduced glutathione 1 200 mg,intravenous drip once a day.Observation group of patients were on the basis of conventional treatment,given intravenous infusion of Xiyanping injection 250 mg,once a day.Patients in both groups were treated for 3 weeks.Two groups of patients before and after treatment serum alanine aminotransferase(ALT),aspertate aminotransferase(AST),total bilirubin(TBIL),indirect bilirubin(IBIL),C-reactive protein(CRP),calcitonin(PCT),tumor transformation factor-α(TNF-α),interleukin 6(IL-6),interleukin 8(IL-8)and lipopolysaccharide(LPS)content.Results Compared with the control group,the total clinical effective rate of the observation group was improved(P<0.05).After treatment,the levels of serum ALT,AST,TBIL,CRP,PCT,LPS,TNF-α,IL-6 and IL-8 in the observation group were lower than those in the control group(P<0.05).Conclusion Xiyanping injection combined with reduced glutathione in the treatment of acute liver injury after severe burns is safe and effective,and can significantly reduce the level of inflammation.
