少年儿童结肠镜检查前肠道准备不良的相关因素分析

Study on related factors of inadequate bowl preparation in children and adolescents before colonoscopy

目的探讨少年儿童结肠镜检查术前肠道准备不良的相关因素,为提高肠道准备质量提供参考依据。方法选择2013年6月-2017年12月在南方医科大学附属小榄医院接受结肠镜检查的128例患者(年龄4～17岁)作为研究对象,根据结肠镜术前肠道准备效果分为肠道准备不良组及肠道准备良好组,记录患者的临床资料,包括性别、年龄、肠道基础性疾病、首次排便时间、服药总时间、服药时机和是否完全服完泻药等。采用单因素分析法及多因素Logistic回归分析法筛选肠道准备不良的危险因素。结果该组患者肠道准备不良发生率为35.16%(45/128)。单因素分析结果显示,年龄、便秘、首次排便时间、服药总时间和是否完全服完泻药是肠道准备不良的危险因素(P <0.05)。多因素Logistic回归分析结果显示,便秘、首次排便时间大于1 h和未能完全服完泻药是肠道准备不良的独立危险因素。结论少年儿童结肠镜检查前肠道准备不良发生率较高,便秘、首次排便时间大于1h和未能完全服完泻药是肠道准备不良的重要因素。

Objective To investigate the related factors of inadequate bowl preparation in children and adolescents before colonoscopy, so as to provide a valuable reference for bowl preparation. Methods 128 children aged 4 to 17 years underwent colonoscopy from June 2013 to December 2017 were selected. According to the quality of bowel preparation, they were divided into inadequate bowel preparation group and adequate bowel preparation group. Clinical data of sex, age, intestinal basic diseases, the first defection time, total time of taking drugs, medication time, incomplete intake of preparation drugs were recorded. Related factors were analyzed by single factor and multiple logistic regression analysis. Results The incidence of inadequate bowel preparation was 35.16%(45/128). Single factor analysis showed that age, constipation, the first defection time, total time of taking drugs and incomplete intake of preparation drugs were risk factors for inadequate bowel preparation. Multivariate analysis showed that constipation, the first defection time more than one hour, and incomplete intake of preparation drugs were independent risk factors for inadequate bowel preparation. Conclusion Children and adolescents undergoing colonoscopy will have higher incidence of inadequate bowel preparation. Constipation, the first defection time more than one hour, and incomplete intake of preparation drugs are all important factors for inadequate bowel preparation.
