Factors Affecting the Quality of Life of Elderly Patients with Urinary Incontinence in Institutions Combining Medical Care with Nursing Care

目的:调查医养结合型机构内影响尿失禁老年人生活质量的相关因素,提高机构内尿失禁老年人的生活质量。方法:采用自设问卷、国际尿失禁咨询委员会尿失禁问卷表简表(ICI-Q-SF)、尿失禁生活质量量表(I-QOL)对1家医养结合型机构的355名住院老年人进行调查,采用多元逐步回归方法分析机构内217名尿失禁老年人生活质量影响因素。结果:机构内尿失禁老年人生活质量得分为(65. 84±26. 72)分,各项目中社交活动方面得分最低为(17. 78±5. 84)分;婚姻状况、医疗付费方式、生活自理能力、饮食是影响尿失禁老年人的总体生活质量的独立因素;医疗付费方式和每日饮水量是尿失禁老年人生活质量限制性行为方面的独立影响因素;医疗付费方式、婚姻状况、饮食、生活自理能力和对机构的满意度是尿失禁老年人生活质量心理社会方面的独立影响因素;医疗付费方式、婚姻状况和生活自理能力是尿失禁老年人生活质量社交活动受限方面的独立影响因素。结论:机构内尿失禁老年人的生活质量得分较低,为提高其生活质量,除早期治疗外,还需要机构针对不同的老年人采取个体化的指导。

Objective:To investigate the related factors affecting the quality of life of elderly patients with urinary incontinence in institutions combining medical care with nursing care,and to improve the quality of life of elderly patients with urinary incontinence in institutions.Methods:A self-designed questionnaire,the incontinence questionnaire of the international incontinence advisory committee(ICI-Q-SF)and the incontinence quality of life scale(I-QOL)were used to investigate 355 hospitalized elderly people in a hospital integrating medical care and nursing.The influencing factors of the quality of life of 217 incontinence elderly people in the hospital were analyzed by multiple stepwise regression method.Results:The score of quality of life of elderly patients with urinary incontinence in the institution was( 65.84 ±26.72),and the score of social activities in each item was the lowest (17.78±5.84).Marital status,mode of medical payment,self-care ability and diet are independent factors affecting the overall quality of life of the elderly with urinary incontinence;The mode of medical payment and daily water consumption were independent factors affecting the quality of life restrictive behaviors in elderly patients with urinary incontinence;Medical payment method,marital status,diet,self-care ability and satisfaction with institutions are independent factors affecting the quality of life of the elderly with urinary incontinence in terms of psychosocial factors;Medical payment method,marital status and self-care ability were independent factors affecting the quality of life and social activities of elderly patients with urinary incontinence.Conclusion:The quality of life score of urinary incontinence elderly palients in institutions is relatively low.In order to improve the quality of life of incontinence elderly,in addition to early treatment,institutions also need to take individualized guidance for different elderly people.
