食管引流型喉罩在腹腔镜胆囊手术麻醉中的应用效果观察

Application of Esophageal Drainage Laryngeal Mask in Anesthesia of Laparoscopic Cholecystectomy

目的观察腹腔镜胆囊手术麻醉中的食管引流型喉罩应用效果。方法选取该院2016年8月—2018年8月收治的74例腹腔镜胆囊手术治疗患者为研究案例,根据随机数字表法分组。对照组37例,采用气管插管麻醉,观察组37例,采用食管引流型喉罩麻醉。对两组不同阶段血液流动力学指标、术后并发症及围手术指标进行观察对比。结果对照插管时、拔管时平均心率及脉动圧均明显超过观察组,比较差异有统计学意义(t=17.387、17.482、10.233、7.638,P=0.000﹤0.05)。对照组总并发症发生率明显超过观察组,比较差异有统计学意义(χ~2=4.563,P=0.032﹤0.05)。观察组平均拔管时间与清醒时间均明显小于对照组,比较差异有统计学意义(t=9.276、9.837,P=0.012、0.010﹤0.05);观察组与对照组平均麻醉时间无明显差异,差异无统计学意义(t=3.262,P=0.063﹥0.05)。结论在腹腔镜胆囊手术麻醉过程中,食管引流型喉罩应用效果较好,值得院方借鉴。

Objective To observe the application effect of esophageal drainage laryngeal mask in anesthesia of laparoscopic cholecystectomy. Methods 74 cases of laparoscopic cholecystectomy patients admitted to our hospital from August 2016 to August 2018 were selected as study cases and grouped according to the random number table method. Thirty-seven patients in the control group were anesthetized with endotracheal intubation, and 37 patients in the observation group were anesthetized with esophageal drainage laryngeal mask. The blood flow mechanical indexes, postoperative complications and perioperative indexes of the two groups at different stages were observed and compared. Results Compared with the observation group, the average heart rate and pulse pressure at intubation and extubation were significantly higher than those in the observation group, less than 0.05 except for statistically significant differences (t=17.387, 17.482, 10.233, 7.638, P=0.000<0.05). Further less than 0.05, the total complication rate of the control group was significantly higher than that of the observation group, with statistically significant difference (χ^2=4.563, P=0.032<0.05). Less than 0.05, the mean extubation time and waking time of the observation group are less than that of the control group, and the difference is statistically significant (t=9.276, 9.837, P=0.012, 0.010<0.05). The average anaesthetic time of the observation group and the control group showed no significant difference, and the comparison showed no statistical significance (t=3.262, P=0.063﹥0.05). Conclusion The application of esophageal drainage laryngeal mask in the anesthesia of laparoscopic cholecystectomy is effective and worthy of reference.
