血、尿Smad2和TGF-β1与IgA肾病尿蛋白及内生肌酐清除率之间关系的研究 被引量:8

The Relationship Between Smad2 and TGF-β1 in Blood and Urine with IgA Nephropathy Urinary Protein and Endogenous Creatinine Clearance

摘要 目的检测分析血、尿Smad2、TGF-β1与IgA肾病患者尿蛋白定量及内生肌酐清除率(CCR)之间的关系。方法分别采集50例IgA肾病患者(IgAN组)及50例无肾脏疾病者(对照组)血和24h尿标本,采用免疫比浊法检测尿蛋白、酶联免疫吸附法检测血、尿肌酐,并计算患者CCR;采用Westernblot方法观察血、尿Smad2和TGF-β1表达情况。结果IgAN组与对照组相比,24h尿蛋白(P<0.01)、血肌酐(P<0.01)水平均高于对照组,CCR(P<0.01)低于对照组,出现肾功能轻度损伤。血、尿Smad2及TGF-β1水平均较对照组增高(P<0.01)。尿Smad2、TGF-β1与尿蛋白定量之间的存在一定正相关性(r分别为0.351,0.474,P<0.05)。但血清Smad2、TGF-β1与尿蛋白定量无明显相关性(P>0.05),血尿Smad2、TGF-β1与CCR无明显相关性(P>0.05)。结论IgAN患者血尿Smad2及TGF-β1均增高,尿液Smad2及TGF-β1与尿蛋白定量有一定相关性。 Objective To investigate the relationship between Smad2 and TGF-β1 in blood and urine and urinary protein quantification and endogenous creatinine clearance(CCR)in patients with IgA nephropathy.Methods The blood and 24-hour urine samples of 50 patients with IgA nephropathy(the IgAN group)and 50 patients without renal disease(the control group)were collected.Urine protein was detected by immunoturbidimetry,also blood and urine creatinine were detected by enzyme-linked immunosorbent assay,and the CCR was calculated.The expressions of Smad2 and TGF-β1 in blood and urine were measured by western blot.Results The 24-hour urine protein(P<0.01)and serum creatinine(P<0.01)levels were higher in the IgAN group than those in the control group.The CCR(P<0.01)in the IgAN group was lower than that in the control group.The renal function was mildly damaged in the IgAN group.The levels of Smad2 and TGF-β1 in blood and urine of the IgAN group were higher than those of the control group(P<0.01).There was a positive correlation between urinary smad2,TGF-β1 and urine protein quantitation( r =0.351,0.474,P <0.05),while there was no significant correlation between serum smad2,TGF-β1 and urine protein quantitation(P>0.05).There was no significant correlation between blood and urine Smad2,TGF-β1 and CCR(P>0.05).Conclusion The levels of Smad2 and TGF-β1 in blood and urine of IgAN patients are increased,and urine Smad2 and TGF-β1 could be correlated with urine protein quantitation.
