977例老年髋骨骨折住院患者治疗现状的分析

Analysis of treatment status of 977 elderly patients with hip fractures

目的:为老年髋骨骨折防治方面提供更全面的资料,为临床工作提供指导和帮助,并为患者提供较为经济的治疗建议。方法:收集中南大学湘雅二医院2012年1月-2017年12月住院治疗的250岁髋骨骨折患者的病例资料,进行统计分析。结果:本组共977例髋骨骨折住院患者纳入研究,男性320例,女性657例,男性与女性患者数之比为1:2.05,全组平均年龄(73.47±10.67)岁,包括股骨粗隆间骨折218例,股骨颈骨折759例(1:3.48);毎个年龄段女性的患者数量都多于男性,骨折类型和治疗方式在不同年龄段患者中的分布差异均有统计学意义(P<0.001),不同年龄段患者的住院,费和住院时间差别存在统计学意义(P<0.05);采用髋关节置换手术患者比内固定术的住院费用高(P<0.001),手术中采用全麻的患者住院费用多于局部麻醉组(P<0.005)。结论:本院老年髋骨骨折住院患者人数逐年增加,女性和70-79岁年龄段的人群居多,全就关节置换术和内固定术为主要的手术方式,从长期康复来看,髋关节置换术是较为经济飭治疗方式。另外,患者住院时间和住院费用随着年龄的增长也随之增加,这为进一步改善髋骨骨折的治疗策略提供了线索。

Objective: To provide more comprehensive information on the prevention and treatment of hip fracture in the elderly, provide guidance and help for clinical work , and provide patients with more economical treatment recommendations. Methods: The data of patients with hip fractures aged 50 years or older who were hospitalized from January 2012 to December 2017 in Xiangya Second Hospital of Central South University were collected for statistical analysis. Conclusion: The number of hospitalized patients with hip fractures in our hospital had increased year by year. Women and 70 - 79 age groups are the most common. Total hip arthroplasty and internal fixation are the main surgical methods. From the perspective of long - term rehabilitation, hip replacement surgery It was a more economical treatment. In addition, patient hospitalization and hospitalization costs increase with age, which provides clues to further improve the treatment strategy for hip fractures.
