吉尼环用于第二次剖宫产术后育龄期妇女的临床研究

Clinical study on application of Gyneflex IUD in women of childbearing age with secondary cesarean section

目的为了给第二次剖宫产术后的育龄期妇女提供一个高效、舒适、依从性好的长效避孕方法并尽可能减少意外妊娠率及人工流产率,分析应用吉尼环(Geneflex IN/S IUD)的价值和意义。方法采用抽签的方法从2018年1~3月来我院就诊的第二次剖宫产术后无生育要求的育龄期妇女中选取100例作为研究对象并按照双盲、对照、随机原则的相关要求将其分为观察组和对照组各50例,其中上圆形金属节育环避孕的患者设为对照组(圆形金属节育环组),应用吉尼环(Geneflex IN/S IUD)的患者设为观察组(吉尼环组),比较不同节育措施对两组上环后的脱落率、因症取出率、有效率以及不良反应发生情况的影响。结果研究数据显示,观察组妇女上环后的脱落率、因症取出率、有效率依次为4.00%、6.00%和90.00%,而对照组妇女上环后的脱落率、因症取出率、有效率依次为16.00%、14.00%和70.00%,观察组避孕有效率明显增加而上环后脱落率和因症取出率明显降低,组间比较差异有统计学意义(P <0.05);和对照组相比观察组妇女各不良反应(经期延长、下腹痛、异常出血及经量增多)发生率明显降低且组间比较差异有统计学意义(P<0.05)。结论吉尼环应用于第二次剖宫产术后的育龄期妇女中不仅可以作为一种安全、可靠、有效的避孕措施,同时还可以减少各种不良反应如异常出血、经量增多等发生率,值得临床应用。

Objective To analyze the application value and significance of Gyneflex IUD(Geneflex IN/S IUD) to provide a long-term and effective contraceptive method with high efficiency,comfort and good compliance for women of childbearing age after the second cesarean section and to reduce the rate of unintended pregnancy and induced abortion as much as possible. Methods 100 women of childbearing age who had no fertility requirements after the second cesarean section from January to March 2018 in our hospital were selected as research objects by the lot method.According to the requirements of double-blind,controlled and randomized principles,they were divided into the observation group and the control group,with 50 cases in each group.Among them,patients with round metal IUD ring for contraception were used as the control group(round metal IUD ring group) and patients with the Gyneflex IUD(Geneflex IN/S IUD) were set as the observation group(the Gyneflex IUD group).The effects of different measures for contraception on the shedding rate of rings,the rate of removal due to disease,the effective rate and the occurrence of adverse reactions were compared. Results According to the study data,the shedding rate of rings,the rate of removal due to disease and the effective rate of women in the observation group were respectively 4.00%,6.00% and 90.00%.The shedding rate of rings,the rate of removal due to disease and the effective rate of women in the control group were respectively 16.00%,14.00% and 70.00%.The effective rate of contraception of the observation group was significantly increased while the shedding rate of rings and the rate of removal due to disease were significantly decreased.The difference between the groups was statistically significant(P < 0.05).Compared with the control group,the incidence of adverse reactions(prolonged menstrual period, lower abdominal pain,abnormal bleeding and increased menstruation volume) was significantly lower in the observation group.The differencea between the groups were statistically significant(P < 0.05). Conclusion Gyneflex IUD can be used not only as a safe, reliable and effective contraceptive measure in women of childbearing age after the second cesarean section, but also can reduce the incidence of adverse reactions such as abnormal bleeding and increased menstruation volume,which is worthy of clinical application.
